As the dose steps up (typically every four weeks, guided by your prescriber) the gut reacts again. Vomiting that starts a day or two after an injection and fades within a week is generally the body adjusting, not a sign that something has gone wrong. That said, it can feel genuinely miserable, and it is worth taking it seriously rather than just pushing through.
Eating large meals, fatty or very rich food, or lying down shortly after eating all make post-dose nausea and vomiting worse. Smaller portions, slower eating, and waiting upright for thirty minutes after meals tend to help. First, stop eating for a short while and sip plain water or an oral rehydration solution in small amounts. Gulping liquid on a revolted stomach almost always makes things worse. Cold or room-temperature drinks are usually better tolerated than hot ones.
Rest upright rather than lying flat. If the vomiting is linked to a recent injection (say, within 48 hours of your dose) it is most likely semaglutide doing what semaglutide does. Give it time before assuming anything more serious is happening.
Do not adjust your next injection timing or skip a dose without speaking to your prescriber first. On the diet side, bland foods (plain crackers, rice, dry toast) are gentler than protein-heavy or spiced meals when symptoms are at their worst.

You have severe abdominal pain that radiates to your back, with or without vomiting. In January 2026 the MHRA issued a Drug Safety Update highlighting acute pancreatitis as a recognised, if uncommon, risk with GLP-1 medicines, the symptom to watch for is exactly this kind of intense, persistent stomach pain, and it needs same-day assessment. You cannot keep any fluid down for more than 24 hours. Dehydration from sustained vomiting can strain the kidneys, and this becomes a medical issue quickly. Signs of significant dehydration include dark urine, dizziness on standing, or a dry mouth that water does not fix.
You notice signs of an allergic reaction (swelling of the face, lips or throat, difficulty breathing, a rapid heartbeat) alongside vomiting. This is rare but requires 999, not a wait-and-see approach.
You have diabetes and your blood glucose is behaving unusually. Persistent vomiting disrupts food intake and can make glucose management unpredictable. Call your diabetes team or GP.

Some people find vomiting happens specifically after meals rather than in the hours following an injection. This is usually related to eating more than the slowed stomach can comfortably handle, a very common adjustment problem in the first month of treatment, and one that catches people off guard because their appetite signals have changed but their portion habits have not yet caught up.
The fix is almost always eating less at one sitting and more slowly. Chewing thoroughly, putting cutlery down between bites, and stopping well before feeling full are practical habits that experienced prescribers routinely recommend.
