What the trial evidence says about gradual dose escalation

The STEP 1 trial, published in the New England Journal of Medicine, followed 1,961 adults with obesity or overweight and at least one weight-related condition for 68 weeks. Participants reached and stayed at the 2.4mg maintenance dose, but only after working through a structured escalation that included the intermediate doses. The researchers found that the stepwise approach was central to tolerability: gastrointestinal side effects, mainly nausea, were most common in the first weeks at a new dose and typically settled before the next increase.

The 4mg dose sits at roughly the halfway point in that escalation. Its job is not to produce significant weight loss on its own but to prepare the body for the doses above it. Staying at 4mg for the prescribed period before moving to 1.7mg (note: the full schedule runs 0.25 → 0.5 → 1mg → 1.7mg → 2.4mg; 4mg is not an official Wegovy step, see the note below) gives the GI tract time to adapt to the medicine's effects on gastric motility. It is worth being precise here. The Wegovy injection titration schedule does not include a 4mg step, the licensed injection doses are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, which is the maintenance dose patients work toward through the full injection schedule. The 4mg tablet step: what it means in the oral semaglutide schedule

The Wegovy tablet was approved by the MHRA on 11 June 2026 as the first oral GLP-1 medicine licensed in the UK for weight management. Its titration is distinct from the injection: patients start at 1.5mg daily, move to 4mg after a minimum of one month, then to 9mg, and eventually to the 25mg maintenance dose, again, at least one month at each level. The 4mg tablet is therefore the second step, reached after the body has had time to settle on the 1.5mg dose.

The trial behind the oral formulation (OASIS 4, 307 adults, 64 weeks) reported around 13.6% average weight loss versus around 2.4% on placebo, regardless of adherence. Among participants who adhered fully to treatment, the figure was closer to 17%, though that higher number applies specifically to the fully adherent group and should not be taken as a typical expectation. Those findings informed the MHRA's approval of this oral route and are the clinical basis for the graduated titration schedule the 4mg dose belongs to.

Taking the tablet correctly matters at every step. It is taken first thing in the morning, on a completely empty stomach, with a small amount of plain water (up to around 120ml). Side effects to be aware of at this stage

Most people find that GI side effects are more noticeable at the start of a new dose step. At 4mg (whether as a tablet step or in any titration context) common effects include nausea, loose stools, constipation, indigestion and burping. These typically ease within the first one to two weeks as the body adjusts. Eating smaller meals, avoiding high-fat foods, and staying well hydrated all help.

The MHRA issued a Drug Safety Update in January 2026 flagging acute pancreatitis as an infrequent but serious risk with GLP-1 medicines: severe stomach pain that radiates to the back, with or without vomiting, warrants prompt medical attention. Other symptoms to take seriously include signs of an allergic reaction, gallbladder pain, or significant dehydration from prolonged GI illness.
