GLP-1 is a hormone your gut releases naturally after eating. Its job, in short, is to tell the pancreas to produce insulin and to stop it over-releasing glucagon, the hormone that raises blood sugar between meals. Wegovy is a GLP-1 receptor agonist, meaning semaglutide mimics that signal continuously, not just at mealtimes.
The glucose-lowering effect that follows is glucose-dependent. Insulin secretion only ramps up when blood sugar is actually elevated, which is why Wegovy alone rarely causes hypoglycaemia in people without diabetes. The body's own feedback loop stays intact. Semaglutide also slows gastric emptying, so carbohydrates reach the bloodstream more gradually, flattening the sharp post-meal glucose peaks that tend to precede cravings and energy crashes.

The STEP 1 trial (68 weeks, semaglutide 2.4 mg versus placebo in adults with obesity but without diabetes) produced an average body-weight reduction of around 15%. Published in the New England Journal of Medicine (STEP 1, 2021), these results have informed NICE's guidance on semaglutide for weight management. The weight loss itself contributes: less visceral fat means improved insulin sensitivity independent of the direct GLP-1 effect. The two mechanisms reinforce each other.
For people who already have type 2 diabetes, the picture is more pronounced. Semaglutide was originally developed as a diabetes treatment (Ozempic uses the same molecule at lower doses for that purpose) and the blood-sugar reductions in diabetic populations are clinically significant. People without diabetes starting Wegovy for weight management rarely need to monitor blood glucose formally. The glucose-dependent mechanism described above acts as a natural ceiling. That said, symptoms worth knowing about include shakiness, sweating, confusion, or sudden hunger out of proportion to the situation, these can occasionally occur, particularly if eating very little during the early weeks of treatment.
The picture changes if you are already taking insulin or a sulfonylurea. Combining Wegovy with these medicines raises the risk of blood sugar going too low, and dose adjustments are often needed. If you experience severe stomach pain that reaches through to your back (with or without vomiting) seek urgent medical attention. The MHRA's January 2026 Drug Safety Update for GLP-1 medicines identified acute pancreatitis as a known, infrequent but serious risk; it is not a blood-sugar symptom, but it can be mistaken for one.
