The question comes up because two things happen at once. First, semaglutide acts on GLP-1 receptors in the gut wall and slows the rate at which food moves from the stomach into the small intestine. Not everyone starting Wegovy needs an iron supplement. A baseline blood test, ideally including serum ferritin (stored iron) as well as haemoglobin, gives you and your prescriber a starting point. Ferritin can be low before haemoglobin drops, so catching it early matters. Groups at higher baseline risk include people who menstruate, those with a history of iron-deficiency anaemia, people with inflammatory bowel conditions that affect absorption in the small intestine, and anyone whose diet was already light on iron-rich foods.
