Wegovy's approval for weight loss in the UK, explained

What exactly was approved, and when?

The original UK licence for Wegovy covered the once-weekly 2.4mg subcutaneous injection, taken via pre-filled pen. Novo Nordisk's application was assessed by the MHRA against its evidence from the STEP clinical programme, including the STEP 1 trial published in the New England Journal of Medicine, which reported an average body-weight reduction of around 15% over 68 weeks at the 2.4mg maintenance dose in adults with obesity.

The approval process matters because it establishes what the medicine is licensed to treat, in whom, and at what doses. For Wegovy, the licence sits squarely with weight management alongside a reduced-calorie diet and increased physical activity, not as a standalone solution. Patients start at 0.25mg and titrate upward at roughly four-weekly intervals, reaching 2.4mg as the standard maintenance dose.

What has the MHRA approved since the original Wegovy pen?

Wegovy's regulatory story did not stop with the initial injection licence. Three significant expansions have followed.

First, the MHRA authorised a higher 7.2mg maintenance dose. This was initially delivered as three 2.4mg pens per week; then on 14 April 2026 the regulator approved a dedicated single-dose 7.2mg pen, one pre-set weekly injection with an auto-dosing mechanism and a built-in covered needle that locks after use. That pen is approved for adults with a BMI of 30 or above only. The titration schedule still starts at 0.25mg; 7.2mg is the ceiling. Second, on 11 June 2026 the MHRA approved Wegovy tablets, making the UK the first country in Europe to license an oral GLP-1 medicine for weight management. The tablet uses a different absorption mechanism and reaches a 25mg daily maintenance dose. Third, from 3 July 2026 the MHRA granted a conditional approval for semaglutide (Wegovy) in metabolic dysfunction-associated steatohepatitis (a form of progressive fatty liver disease) in adults with moderate-to-advanced fibrosis. Who qualifies under the Wegovy weight-loss licence?

The licensed eligibility criteria for Wegovy in the UK are: adults aged 18 or over with a BMI of 30 or above, or a BMI between 27 and 29.9 alongside at least one weight-related condition such as high blood pressure, type 2 diabetes, dyslipidaemia or obstructive sleep apnoea. Lower BMI thresholds apply for some ethnic backgrounds under UK clinical guidance. The 7.2mg pen licence adds a tighter constraint: BMI of 30 or above only, not the 27-plus-comorbidity bracket.

Licence eligibility is the floor, not the ceiling. A prescriber assesses your full medical picture (current medicines, medical history, and the specific medicine in question) before any prescription is issued. NICE has also published its own recommendation for semaglutide under technology appraisal TA875, setting out the criteria for NHS use. Those criteria are stricter than the licensed population and require use within a specialist weight management service for a maximum of two years. What does approval mean for people who want to start treatment?

MHRA approval confirms that the medicine met the regulator's standards for quality, safety and efficacy in the licensed population. It does not mean the medicine is available from every pharmacy, on the NHS, or suitable for every person who wants it.

On the NHS, Wegovy is available via specialist weight management services under TA875; waiting lists are commonly lengthy, and the eligibility criteria are narrower than the full licensed population.
