Wegovy click dosage: what each number on the pen dial actually means

You injected and heard several clicks, that's normal, and here's why

A lot of people finishing their first injection pull the pen away wondering whether it worked. They counted the clicks, found no consistent number across injections, and started worrying. That concern is understandable. The audible and tactile clicks from a Wegovy pen come from the drive mechanism inside the device as the plunger advances, not from a click-per-unit counting system the way some insulin pens work. The number of clicks you hear can vary depending on how firmly you hold the pen against your skin, the injection angle, and the natural variation in the spring mechanism. None of that variation changes how much semaglutide has been delivered.

What actually determines your dose is the dose window: the small clear window on the side of the pen body where a number appears when you turn the dose dial. That number (0.25, 0.5, 1.0, 1.7 or 2.4 (or 7.2 on the newer pen)) is the dose in milligrams your prescriber has set for this stage of your treatment. Confirm that number before every injection. The clicks are reassuring confirmation that the mechanism is moving, but they are not a measurement.

The dose ladder: what each stage is for, and why rushing it backfires

Wegovy's titration schedule exists for a specific reason: semaglutide's most common side effects are gastrointestinal, and starting at the maintenance dose would, for most people, produce significant nausea. The 0.25 mg starting dose is a settling-in period, your gut is adjusting to the medicine, not yet receiving a therapeutic weight-management dose. Most people notice relatively little appetite change at 0.25 mg, which occasionally makes them think the medicine isn't working. It is working; it's just doing the tolerability job first.

From there, the schedule typically moves to 0.5 mg, then 1.0 mg, then 1.7 mg, with each step around four weeks apart, before reaching the 2.4 mg maintenance dose. Your prescriber may extend time at any step if side effects need more time to settle. In April 2026 the MHRA approved a dedicated single-dose 7.2 mg pen for adults with a BMI of 30 or above, following trial evidence reporting around 20.7% average weight loss over 72 weeks, though that dose is the maximum, not a starting point, and only your prescriber can determine whether it is suitable for you.

Skipping steps or dialling to a higher dose window than prescribed is not safe and, in practice, will likely intensify nausea without meaningfully improving outcomes. How to read the dose window correctly before each injection

The practical sequence matters. Before you inject, check the dose window shows the number your prescriber has confirmed for this stage. Inspect the liquid, it should be clear and colourless; do not use it if it looks cloudy or contains particles. Attach a new needle, prime if indicated in your leaflet, choose your injection site (abdomen, thigh or upper arm, rotated each week), and press the pen firmly against the skin. Hold the button down and keep the pen pressed in place until the dose window shows zero and a few more seconds have passed, your leaflet gives the exact count. Only then withdraw.

There are a handful of situations where reaching out promptly is the right move. If you are unsure whether an injection delivered fully, do not guess, call your pharmacy or prescriber the same day.
