The pen is in your hand. You flip it over and there's a list of names (disodium phosphate dihydrate, phenol, propylene glycol) and you wonder whether any of it affects you. This is worth a minute of your time, especially if you have allergies or are switching from another medicine. The short answer is that the formulation is straightforward, but understanding what each component does helps you use the treatment with confidence.

Semaglutide is the working part. It is a synthetic analogue of human GLP-1, a hormone released by the gut after eating. Semaglutide binds to GLP-1 receptors in the brain and pancreas, reducing appetite and slowing how quickly the stomach empties. The result, across the clinical trial programme for Wegovy, was an average body-weight reduction of around 15% over 68 weeks at the 2.4 mg maintenance dose, as reported in the STEP 1 trial published in the New England Journal of Medicine. The other components in the pen do none of that work directly, they exist to protect semaglutide and make the injection tolerable.

One practical habit worth building: before each new pen, check the solution through the pen window. It should be clear and colourless to very slightly yellow. What the inactive ingredients actually do

Disodium phosphate dihydrate and the water for injections act as the buffer and solvent, maintaining a stable pH so the semaglutide molecule stays intact from manufacture through to the moment it enters your tissue. Small shifts in pH can denature proteins, so this buffer matters more than its unglamorous name suggests.

Phenol is there as a preservative. Multi-dose pens carry a small microbiological risk each time the pen is used; phenol prevents bacterial or fungal growth in the solution over the weeks of use. The concentration is low and within established safety limits for injectable medicines, as documented in the Wegovy SmPC on the electronic Medicines Compendium.

Propylene glycol helps keep the formulation homogeneous and assists with solubility. It is widely used in injectable medicines at concentrations far below any level of concern. None of these excipients contains gluten, lactose, or animal-derived gelatine, which is relevant for patients with specific dietary considerations. The pen itself is made of plastic and stainless steel; there is no latex in the needle cap.

Does the formulation change across Wegovy dose strengths?

The inactive components stay constant across all Wegovy pen strengths (0.25 mg, 0.5 mg, 1.0 mg, 1.7 mg, and 2.4 mg, with the newer 7.2 mg pen approved by the MHRA in April 2026 following the same formulation principles). What changes is the volume of solution delivered per injection. Because the concentration is fixed at 1 mg/ml, a 2.4 mg dose means a larger injection volume than the 0.25 mg starter dose. The titration itself begins at 0.25 mg and moves through four steps to 2.4 mg (or up to 7.2 mg for eligible patients), with each step spaced roughly four weeks apart by the prescriber. When the components become clinically relevant

For most people, the formulation raises no issues at all. The cases where it does matter are narrower: a documented phenol allergy, a propylene glycol sensitivity, or a situation where a patient is also on medicines that interact at the GLP-1 pathway rather than at the excipient level.
