Wegovy in England: how to access it, what it costs, and what to expect

Step 1: understand what Wegovy is licensed to do in England

Wegovy is semaglutide, a GLP-1 receptor agonist made by Novo Nordisk. Its UK licence covers weight management in adults, alongside a reduced-calorie diet and increased physical activity. The standard dosing pathway runs from 0.25mg up to 2.4mg weekly, titrated by your prescriber roughly every four weeks. In January 2026, the MHRA approved a higher 7.2mg maintenance dose, and a dedicated single-dose 7.2mg pen followed in April 2026, so the ceiling has shifted upward since the medicine first launched.

It is not the same product as Ozempic. Both contain semaglutide, but Ozempic holds a licence for type 2 diabetes, not weight management. NICE formally recommended Wegovy for NHS use in England under technology appraisal TA875 (published March 2023). That recommendation came with conditions that determine who can receive it through the health service, which is where it gets complicated.

Step 2: check whether you meet the NHS England criteria

NICE TA875 limits NHS Wegovy to adults with at least one weight-related comorbidity and a BMI of 35 or above (or 30 to 34.9 where specialist-referral criteria are met). Ethnic-background thresholds are 2.5 kg/m² lower for people of South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, so the relevant cut-off may be 32.5 or 27.5 to 32.4 respectively.

There is also a time limit built into the NHS route: NICE recommends Wegovy for a maximum of two years, within a specialist weight management service that provides multidisciplinary support. If less than 5% weight loss is achieved after six months at the maintenance dose, continuing treatment is reviewed. This is a meaningful constraint, the NHS is not simply offering the medicine indefinitely to anyone with an elevated BMI.

NHS England's guidance on weight management injections sets out these criteria in full, including the wraparound lifestyle support that must accompany treatment. Meeting the numbers is necessary; it is not by itself sufficient. Waiting lists at specialist services remain long in most parts of England.

Step 3: decide whether a private prescription makes sense for you

Private prescription is the route most people in England currently use, simply because NHS access is rationed and slow. For a private prescription to be legitimate, you still need a clinical assessment by a qualified prescriber, the medicine does not change its status because you are paying privately. It is a prescription-only medicine, full stop.

The private licensed eligibility broadly mirrors the SmPC: adults with a BMI of 30 or above, or 27 or above with a weight-related condition such as high blood pressure, high cholesterol or obstructive sleep apnoea. A prescriber considers the full clinical picture, not just the number on the scales. Step 4: know the safety points before you start

The most commonly reported side effects with Wegovy are gastrointestinal: nausea, loose stools, constipation, indigestion and fatigue tend to cluster around dose increases and usually ease as your body adjusts. Slower titration, smaller meals and staying well hydrated all help, though your prescriber will go through the specifics with you.

Two safety points worth holding onto. First, tell your surgical team and anaesthetist if you are on Wegovy before any planned procedure. Second, if you take the combined oral contraceptive pill, Wegovy (unlike tirzepatide) does not carry the same evidence of reduced pill absorption, but this is worth raising with your prescriber if you have any doubt.

For urgent symptoms (severe stomach pain that spreads to your back, signs of a serious allergic reaction, or symptoms of gallbladder trouble) seek medical help promptly.
