Wegovy as a GLP-1 medicine: what semaglutide actually does in your body

A question our prescribers hear most weeks is whether Wegovy and Ozempic are interchangeable. They contain the same molecule, but they are not the same medicine for regulatory purposes. Ozempic is licensed for type 2 diabetes, not for weight management, and should not be obtained for weight loss. Wegovy's licence is specifically for weight management in adults meeting the BMI criteria above.

What the GLP-1 pathway looks like at Wegovy's doses

Wegovy starts at 0.25 mg weekly, moving through 0.5 mg, 1.0 mg and 1.7 mg before reaching the 2.4 mg maintenance dose, each step roughly four weeks apart, paced by the prescriber to let the body adjust. Most of the gastrointestinal side effects that people notice (nausea, some loose stools, a sense of reflux) are sharpest at the beginning or after a dose increase, and they tend to settle as the body adapts to sustained GLP-1 receptor activity.

In January 2026 the MHRA approved a 7.2 mg maintenance dose of Wegovy, following clinical data showing that the higher dose produced around 20.7% average body-weight reduction over 72 weeks, narrowing the gap with tirzepatide's results at its top dose. A dedicated single-dose 7.2 mg pen was then approved on 14 April 2026. Whether a patient starts on the standard 2.4 mg pathway or is considered for 7.2 mg is a clinical decision made with a prescriber; the underlying GLP-1 mechanism is the same at every dose level, just with a different degree of receptor engagement.

Who the GLP-1 licence actually covers, and where the NHS fits in

The UK licence for Wegovy covers adults with a BMI of 30 or above, or a BMI of 27 to 29.9 alongside at least one weight-related condition such as high blood pressure, raised cholesterol or obstructive sleep apnoea. Lower BMI thresholds apply for some ethnic backgrounds under UK guidance. BMI alone never settles the question, the prescriber looks at the full clinical picture, including any medicines you take, your medical history and any contraindications listed in the Wegovy SmPC on the eMC.

On the NHS, access to Wegovy follows NICE guidance (TA875), which restricts it to specialist weight management services and a maximum duration of two years. Waiting lists at those services are typically long.
