Which route is right for you: NHS or private?

Wegovy is available on the NHS, but only through specialist weight management services and only for patients who meet the criteria set out in NICE's appraisal TA875. Those criteria include a BMI of 35 or above, at least one weight-related health condition, and a place in a multidisciplinary specialist programme. Waiting lists in most areas are long, often many months. The NICE recommendation also caps NHS treatment at two years.

Private prescribing operates under the same clinical standards but bypasses the referral queue. A regulated online pharmacy assesses your eligibility directly. A large number of sellers advertising Wegovy, or medicines described as semaglutide, online and on social media are operating without a UK pharmacy licence. The MHRA has repeatedly warned about counterfeit weight-loss pens sold this way, fake pens have been found to contain insulin, and some people have been hospitalised. In late 2025, enforcement teams seized roughly 20 million doses of illegally traded weight-loss medicines worth an estimated £45 million, and raided a site manufacturing counterfeit tirzepatide pens inside the UK. The MHRA asks anyone who comes across a suspect seller to report them through the Yellow Card scheme.

Whether you meet the eligibility criteria for Wegovy

Wegovy is licensed for adults with a BMI of 30 or above, or 27 or above where at least one weight-related condition is present, for example, high blood pressure, high cholesterol, type 2 diabetes, or obstructive sleep apnoea. For some ethnic backgrounds, UK guidance applies thresholds that are 2.5 kg/m² lower; your prescriber will take that into account. BMI alone is never the whole picture. A prescriber will also look at your medical history, any medicines you take, and whether there are any reasons Wegovy would not be suitable for you.
