Semaglutide works by activating GLP-1 receptors in the brain and gut, slowing gastric emptying and significantly reducing appetite. The result most people are aiming for (eating considerably less) has a side effect that surprises some: your body is suddenly running on a much smaller fuel supply. If your calorie intake drops sharply in the first weeks, fatigue is a natural consequence. It is not a sign the medicine is harming you; it is a sign your system is adjusting to a new normal.
There is also a direct pharmacological element. The NHS semaglutide medicines page lists fatigue and headache alongside the more familiar gastrointestinal effects, confirming they are recognised reactions rather than rare outliers. Headaches, which often accompany the early tiredness, are typically linked to the same appetite suppression and reduced fluid intake. Drink more water than feels necessary. That part really does help.
Dose increases tend to restart the pattern. Someone who felt fine at 1mg may notice lethargy again after moving to 1.7mg, then again at 2.4mg. Each step asks your body to readjust, and each adjustment carries its own short window of side effects. Most people find the tiredness at each level lighter and shorter-lived than at the previous one.
For most people, fatigue peaks in the first few days after starting a new dose and eases noticeably within one to two weeks. By the time the next scheduled dose increase arrives, tiredness is rarely still the main complaint, nausea tends to linger longer. If you have been on a stable dose for more than two or three weeks and still feel persistently drained, that is a different conversation to have with your prescriber.
Prolonged lethargy can sometimes reflect inadequate nutrition during treatment. Wegovy suppresses appetite effectively, but the body still needs protein, iron, B vitamins and enough calories to function. People who eat very little (sometimes as little as one small meal a day) can slide into a nutritional shortfall that compounds the tiredness. Dehydration is the other common driver. Nausea and vomiting reduce how much people drink, and some people unconsciously cut fluids along with food. Fatigue from dehydration can become serious quickly, if reduced drinking is accompanied by dizziness, dark urine or confusion, seek medical help rather than waiting for it to pass.
Most lethargy on Wegovy is benign and self-limiting. A handful of patterns deserve a more urgent look.
Severe, persistent stomach pain that radiates to the back, especially alongside vomiting, is not ordinary nausea, it can indicate acute pancreatitis, which the MHRA highlighted in a dedicated Drug Safety Update for GLP-1 medicines in January 2026 as a known but infrequent serious risk. Anyone experiencing that combination of symptoms should seek urgent medical help rather than attributing it to ordinary tiredness.
Signs of significant dehydration (inability to keep fluids down for more than 24 hours, very dark urine, rapid heart rate) also warrant a call to 111 or a GP. Kidney function can be affected by prolonged dehydration, and that has its own fatigue signature that goes beyond the usual post-dose dip.
Fatigue that arrives alongside mood changes, low motivation or thoughts of self-harm should be flagged promptly. These are listed as possible side effects across GLP-1 medicines and are taken seriously by prescribers. The pen goes in the fridge door; the daily habit of drinking a full glass of water before you check your phone matters more than it sounds. Small, consistent habits carry a lot of weight when your appetite is suppressed and your routine has shifted.
Beyond hydration, protein is the most practically useful dietary focus during treatment. It supports muscle retention, keeps energy levels more stable between the small meals most people manage, and reduces the crash that comes from high-carbohydrate eating on very little food. You do not need a dietitian's detailed plan to make this work, swapping half a meal for eggs, Greek yoghurt or fish is enough to notice a difference for most people.
Timing your injection can also help. Some people find weekly doses leave them most tired on the day after injection; others hardly notice the pattern at all. Tracking your energy across a few weeks gives your prescriber genuinely useful information rather than a general complaint of feeling tired.
