The most common assumption people bring to this question is that Wegovy must be suppressing libido the way some antidepressants or hormonal medicines do. That is not what the licensed prescribing information says. The NHS semaglutide medicines page lists the recognised side effects of semaglutide: nausea, vomiting, diarrhoea, constipation, headache, dizziness, fatigue and injection-site reactions. Reduced libido does not appear. Semaglutide works as a GLP-1 receptor agonist, acting on appetite-regulating pathways and slowing gastric emptying; it does not directly interact with sex hormones or the neural pathways that govern sexual desire.
That does not mean everyone's sex drive stays constant throughout treatment. It means the medicine itself is unlikely to be the direct cause when changes do occur. The more productive question is what else is happening in someone's body and life during significant weight loss, because quite a lot is.

Obesity is associated with measurable hormonal disruption. In men, excess adipose tissue converts testosterone to oestrogen through a process called aromatisation, which can suppress circulating testosterone levels significantly. Research published in clinical endocrinology literature consistently links higher BMI with lower free testosterone and reduced sexual function in men. As body fat falls, that aromatisation burden falls with it, and testosterone can recover. For some men on Wegovy, the net hormonal effect of losing 10–15% of body weight is an improvement in sex drive rather than a reduction, the opposite of what people expect.
For women, the picture is similarly complex. Weight loss can improve polycystic ovarian syndrome (PCOS) symptoms, reduce insulin resistance and restore hormonal balance, all of which have downstream effects on sexual wellbeing. Improved energy, reduced joint pain and better sleep (all plausible outcomes of sustained weight loss) contribute to sexual function in ways that are hard to isolate from the medicine's direct pharmacology. The specific effects of Wegovy on libido in women are worth reading if that is your situation, as the hormonal interplay differs.
The STEP 1 trial, published in the New England Journal of Medicine, showed average body-weight reductions of around 15% over 68 weeks with semaglutide 2.4mg alongside lifestyle support. Changes of that magnitude carry real physiological consequences for hormone profiles, consequences the trial's primary endpoints were not designed to measure, but which are biologically plausible and consistent with what patients report.
Several factors that commonly accompany starting Wegovy can affect sex drive, and none of them are permanent. Nausea is the most frequently reported side effect, particularly in the first weeks of each dose step. Feeling persistently nauseous makes intimacy unappealing for straightforward reasons that have nothing to do with hormones. Fatigue is similarly common early in treatment. The psychological weight of changing your diet, adjusting to injections and monitoring your progress creates its own cognitive load.
Caloric restriction (a central part of weight management alongside any medicine) can temporarily reduce energy levels and, by extension, sexual energy. If intake drops sharply, that effect amplifies. Body image shifts during weight loss are also not straightforward. Some people feel more confident quickly; others find the transition unsettling, especially if change is rapid or uneven. Both responses are normal. The evidence base for testosterone improving alongside significant weight loss is solid enough that some clinicians consider weight management a first-line intervention for hypogonadism related to obesity, before considering testosterone replacement.
