You've been on Wegovy for a while. The first few weeks were rough (nausea after most meals, the odd bout of reflux) and you assumed it would pass. For many people it does. But some find that certain effects linger, and not knowing whether that is expected or a signal worth acting on is genuinely unsettling. That uncertainty is what this page is for.
The semaglutide long-term side effects that persist beyond the initial titration period are mostly mild gastrointestinal ones: intermittent nausea, loose stools, constipation, or a tendency to feel full very quickly. The STEP 1 trial, published in the New England Journal of Medicine, followed adults on semaglutide 2.4 mg for 68 weeks. GI events were common early on, declined sharply as the body adjusted, and remained the dominant reported effect class throughout, but most were described as mild to moderate in severity. What the trials cannot yet answer fully is what happens beyond two years of continuous use, because most long-term trial follow-up data does not extend much further. If you are wondering whether Wegovy has long-term side effects that only emerge after extended use, that is not a reason to be alarmed, it is simply honest. The NHS patient information for semaglutide lists the known side effects at every frequency tier, and it is the clearest public reference for checking whether what you are experiencing is recognised.
Gallbladder problems are the long-term concern that comes up most consistently in post-marketing surveillance and trial extensions. Rapid weight loss of any kind increases the likelihood of gallstone formation, and semaglutide-driven weight loss is no exception. Symptoms to watch for include sharp pain in the upper right abdomen, pain after fatty meals, or pain that radiates to the shoulder blade. If those symptoms appear, contact your prescriber or GP promptly rather than waiting for your next scheduled review.
Pancreatitis is rarer but more serious. In January 2026 the MHRA issued a Drug Safety Update for GLP-1 medicines specifically highlighting acute pancreatitis as a known, infrequent but potentially serious side effect. The symptom to act on is severe, persistent stomach pain, particularly pain that reaches through to the back, with or without vomiting. That combination warrants same-day medical attention. For people with type 2 diabetes already at risk of diabetic retinopathy, there is evidence that rapid early improvements in blood sugar can temporarily worsen retinal changes. This is not unique to semaglutide, but it is a reason why a prescriber needs to know your full medical history before treatment starts. Those on Wegovy for weight management without diabetes are not thought to carry the same level of risk, but any new or changed vision symptoms should be reported to a clinician without delay.
Most side effects of long-term Wegovy use are manageable and self-limiting. A small number are not. Seek urgent care (same day, not next available appointment) if you experience any of the following: severe or persistent abdominal pain that spreads to your back; signs of an allergic reaction such as facial swelling, difficulty swallowing or a spreading rash; significant dehydration from prolonged vomiting or diarrhoea; or sudden changes in vision. For symptoms that worry you but are not immediately alarming, your prescriber is the right first contact. A question our prescribers hear regularly is whether staying on Wegovy for years causes the body to lose muscle rather than fat, or whether bones become thinner. If you want a thorough answer to what the long-term side effects of Wegovy are, including effects on body composition, the evidence here is still developing. GLP-1 receptor agonists like semaglutide act primarily on appetite and gastric emptying; they do not directly target muscle. But any sustained calorie deficit, from any cause, risks lean-tissue loss if protein intake and resistance activity are not maintained. That is why clinical guidance consistently recommends a reduced-calorie diet alongside increased physical activity, not simply as a lifestyle add-on, but because the combination protects muscle mass during weight loss.
On bone health, current trial data does not show a significant semaglutide-specific effect on bone mineral density beyond what would be expected from weight loss alone. If you have existing osteoporosis or a history of fractures, that is a specific point to raise with your prescriber, it does not automatically exclude you from treatment, but it shapes the conversation.
