Wegovy Second Month Results: What the Evidence Actually Shows

Most people are still mid-titration in month two, moving from 0.5 mg toward 1.0 mg semaglutide — the therapeutic doses come later.
The STEP 1 trial recorded an average of around 15% body-weight reduction over 68 weeks; month-two results represent a small fraction of that arc.
Side effects are often most noticeable in the first two months, particularly after each dose step — nausea, fatigue and digestive changes tend to ease as the body adjusts.
Wegovy (semaglutide 2.4 mg) is a Black Triangle (▼) medicine under additional MHRA monitoring; every prescription requires clinical assessment.

By the second month of Wegovy, most people are partway through dose titration and starting to feel the medicine work differently from those early weeks. Average weight loss at this stage typically sits in the range of 2–5% of starting body weight, though individual results vary considerably depending on dose reached, diet, and how the body is responding. That range matters: Wegovy is a prescription-only medicine, and how it works for you is something a prescriber monitors and adjusts over time. If you're reading this because your results feel slower than expected, that's one of the most common things our clinical team hears, and the answer is nearly always more nuanced than the headline trial figures suggest.

What month two actually looks like, and why the timeline trips people up

The myth that month two should show dramatic results

There's a persistent idea that weight-loss injections produce rapid, visible change from the first few weeks. Social media reinforces it. The reality is more measured, and understanding why makes month two far less frustrating.

Wegovy's titration schedule exists for a reason. The starting dose of 0.25 mg is not a therapeutic dose; its job is tolerability, giving your digestive system time to adjust to semaglutide before the dose rises. By month two, most people are at 0.5 mg or just reaching 1.0 mg. Neither is the maintenance dose of 2.4 mg. The medicine is still, in clinical terms, building toward its working level.

This means comparing your month-two weight loss to the headline trial figures is comparing the wrong things. The STEP 1 trial's ~15% average reduction, published in the New England Journal of Medicine, was recorded at 68 weeks, well over a year of treatment at full dose. Month two sits near the beginning of that curve, not anywhere near its peak. Letting go of the idea that slow month-two progress means the medicine isn't working is probably the single most useful reframe available.

There's also the question of what's happening physiologically. Semaglutide acts on GLP-1 receptors to slow gastric emptying and reduce appetite signals; that mechanism takes time to exert its full effect at each dose level. Some people notice a clear reduction in appetite within days of a dose increase. Others take a few weeks. Both are within the normal range.

What the evidence says about weight loss across the first two months

Clinical trial data doesn't break results down to a precise 'month two' figure, because the trials were designed around longer endpoints. What the data does show is a consistent pattern: weight loss is gradual in the early weeks and accelerates as doses increase toward maintenance.

Across the STEP programme, participants taking semaglutide 2.4 mg lost roughly 5–7% of body weight by around week 16 to 20, and continued losing through to week 68. Month two (roughly weeks 5–8) is earlier still. If you want to understand how typical early progress compares to your own experience, our guide to Wegovy one-month results sets out what most people see in the first four weeks and how that feeds into what comes next.

Body-weight percentage is a more useful measure than kilograms lost, because it accounts for starting point. Someone starting at 120 kg losing 2.5% has lost 3 kg. That's real, clinically meaningful progress, even if it doesn't feel dramatic on the scale. The NHS's guidance on semaglutide notes that treatment is intended to be gradual and supervised, and that's by design, not a flaw.

Appetite reduction is often the clearest early signal. People frequently report feeling full faster, thinking about food less, and finding it easier to leave food on the plate. Those changes can precede visible scale movement by a few weeks, because the body is also adjusting fluid balance and lean mass alongside fat. Month two is often when that subjective shift starts to feel stable.

For a fuller picture of how results develop beyond this point, the three-month mark tends to show clearer progress as higher doses take effect.

Side effects in month two, and when they deserve attention

Month two is when side effects are often still present, particularly around dose increases. Nausea is the most commonly reported; diarrhoea, constipation, indigestion and fatigue appear in a significant proportion of people too. For most, these settle within a week or two of each dose step.

Eating smaller portions, avoiding high-fat meals, and staying well hydrated all help. These aren't just lifestyle tips alongside the medicine, they're part of how Wegovy is designed to be used, alongside a reduced-calorie diet and increased physical activity.

There are symptoms that warrant prompt medical attention rather than waiting. Severe stomach pain that radiates toward the back, with or without vomiting, should be assessed urgently because it may indicate pancreatitis. The MHRA issued a Drug Safety Update in January 2026 specifically highlighting this as a known but infrequent risk with GLP-1 medicines. Gallbladder symptoms, severe dehydration from persistent vomiting, and any signs of an allergic reaction should also be seen promptly. If you're uncertain, contact your prescriber or call 111.

It's worth knowing that side effects during month two don't predict how the rest of treatment will go. Many people find the second half of titration considerably easier than the first.

When to talk to your prescriber about month-two progress

If you started Wegovy through a private service and feel your progress or side effects deserve a closer look, that's exactly what aftercare support is for. A good prescriber will look at the dose you've reached, how you're tolerating it, any changes in appetite, and whether the titration schedule needs adjusting, not just the number on the scale.

For context on what the full cost of that ongoing support includes, the Wegovy cost page sets out what a transparent private price should cover, including clinical review at every stage. Prescription medicines don't become safer because they're cheaper, and month-two check-ins are part of the value of supervised treatment.

If you haven't started yet and are weighing up whether Wegovy is right for you, our Wegovy overview covers eligibility, how the medicine works, and what the titration schedule looks like from the beginning. A realistic timeline for how long Wegovy takes to work also helps set expectations across all the early months, not just month two.

Treatment is a prescription-only decision made with a clinician. If you want to plan ahead, our breakdown of Wegovy four-month results gives a useful sense of where most people are once the higher doses have been in place for a while. If you're ready to explore whether you're eligible, you can check your eligibility with our prescribers, no waiting list, no referral needed, reviewed the same day.
