Why does Wegovy make you feel tired?

Semaglutide works by activating GLP-1 receptors in the brain and gut, slowing how quickly your stomach empties and reducing appetite signals. That's the therapeutic mechanism, but those same pathways play a role in regulating energy levels too, which is part of why fatigue shows up as a side effect. Your body is recalibrating its relationship with food intake, hormones and digestive rhythm all at once. That takes energy.

There's also a practical side: when your appetite drops sharply, calorie intake often falls with it. If you're eating significantly less than your body is used to, tiredness is a predictable consequence. This doesn't mean the treatment is wrong for you; it means your eating pattern may need some thought. Prioritising protein at each meal and staying well hydrated can make a meaningful difference to how you feel day to day. Light movement (even a short walk) tends to help more than complete rest, once you're not feeling unwell.

How long does tiredness on Wegovy tend to last?

For most people, the tiredness tied to starting Wegovy (or to being intensely fatigued on Wegovy during an increase) peaks within the first two weeks and then begins to settle. The pattern mirrors the broader GI side-effect curve: the body's response is usually sharpest at the point of change, then adapts. By week three or four at a given dose, most people feel considerably more normal.

That said, some people find the fatigue while on Wegovy persists beyond that window, particularly if GI symptoms like nausea or disrupted sleep are also ongoing. If you're not sleeping well because of digestive discomfort, tiredness compounds quickly. Addressing the root cause (whether that's nausea management, eating timing or hydration) usually helps fatigue more than anything else.

A smaller number of people find that tiredness remains an issue even at a settled dose. If that's the case for you, it's worth mentioning to your prescriber before any further dose increase is considered. Fatigue that doesn't improve, or that's accompanied by symptoms like dizziness, pallor or rapid heartbeat, should be assessed rather than waited out. Is this the same tiredness that comes with semaglutide in general?

Yes. Wegovy contains semaglutide, and the fatigue associated with it is the same mechanism seen across semaglutide-based medicines. If you've read about tiredness linked to semaglutide more broadly, it applies here. The difference is context: Wegovy is licensed for weight management at doses up to 2.4mg weekly (and, from April 2026, a 7.2mg maintenance dose approved by the MHRA), while Ozempic is licensed for type 2 diabetes, but the side-effect profile overlaps because the active compound is the same.

The STEP 1 trial, which evaluated semaglutide 2.4mg over 68 weeks and was published in the New England Journal of Medicine, reported fatigue among the recognised adverse events in the treatment group. It was generally mild to moderate and transient, which matches what most people experience in practice. The question of whether Wegovy causes tiredness has a straightforward clinical answer: for a meaningful proportion of users, yes, but it is not a sign the medicine is harming you, and for the majority it resolves without any change to treatment.

When should you tell your prescriber about tiredness on Wegovy?

Most fatigue during Wegovy treatment is self-limiting and manageable. But there are situations where a prescriber should be told promptly rather than just waited out. Contact your clinical team if: tiredness is severe enough to affect your ability to function at work or at home; it is worsening rather than improving after two to three weeks at a stable dose; it is accompanied by severe stomach pain (especially pain that radiates to the back), persistent vomiting, signs of dehydration, or any allergic reaction symptoms.

Severe stomach pain alongside nausea and vomiting can indicate pancreatitis, which is a less common but serious side effect of GLP-1 medicines. The MHRA has issued clear guidance that this needs urgent medical attention. That's not meant to alarm (the risk is real but uncommon, and ordinary tiredness feels nothing like it) but it's the kind of symptom where waiting is the wrong call.
