Wegovy weight loss after 2 months: facts over expectations

Two months in, most people are still titrating — the 2.4 mg maintenance dose is typically reached at around month five, so early results are not the full picture.
Trial data suggest around 15% average body-weight reduction over 68 weeks at the maintenance dose; the two-month point sits early on that curve.
The most common side effects at this stage are gastrointestinal (nausea, constipation and reflux) and tend to be most noticeable during or just after a dose step.
Appetite reduction is usually noticeable by week four to eight, even before results on the scale catch up.

By the two-month mark on Wegovy, most people have completed the 0.5 mg dose and are approaching or settling on 1.0 mg — still well below the 2.4 mg maintenance dose. Average weight loss at this stage is modest in absolute terms, typically in the range of 3–6% of starting body weight, though individual results vary considerably. That figure comes from clinical trial data published in the STEP 1 study in the New England Journal of Medicine, which tracked adults over 68 weeks. Wegovy (semaglutide) is a prescription-only medicine, and any treatment must follow a clinical assessment by a registered prescriber who decides whether it is suitable for you.

What the evidence and timeline actually look like at the two-month point

The biggest misconception: two months is not a fair test of Wegovy

Many people weigh themselves at eight weeks, see a smaller number than they expected, and wonder whether the medicine is working. The most common misunderstanding is treating the two-month point as if it were a definitive result. It is not. Wegovy's titration schedule moves through four doses (0.25 mg, 0.5 mg, 1.0 mg, 1.7 mg) before reaching the 2.4 mg maintenance level, and the prescriber typically holds each step for around four weeks. At eight weeks, most people are on 0.5 mg or just moving to 1.0 mg. That is not a therapeutic plateau; it is a controlled approach designed to reduce the chance of nausea and other gastrointestinal side effects.

The STEP 1 trial, which informed the UK licence and NICE's appraisal of semaglutide (TA875), ran for 68 weeks. The steepest weight-loss curve in that data came after the maintenance dose was established and sustained. Expecting full results at two months is a bit like judging a marathon runner's time at mile four. You are seeing early progress, not the finishing line.

If the scale has barely moved but your appetite is noticeably lower, that appetite shift is the medicine beginning to work. For many people, the behavioural changes (eating smaller portions, feeling full faster, thinking about food less) appear before the weight loss does. That is biologically normal, not a sign of failure.

What weight loss at two months on Wegovy typically looks like

There is no single answer because starting weight, dose reached, diet, activity and individual metabolism all play a role. That said, looking at the trial data and clinical experience together, a reasonable benchmark for two months on Wegovy is somewhere between 3% and 6% of starting body weight. For someone starting at 100 kg, that is roughly 3–6 kg. Some people see more, especially if they were already making dietary changes before starting; some see less, particularly if they have had to slow their titration because of side effects.

Weight loss also does not happen in a straight line. A week of no movement on the scale can follow two weeks of steady progress, and that pattern is common and well documented. Fluid shifts, hormone cycles, constipation (itself a frequent side effect at this stage) and changes in muscle mass all affect what the scales show on any given morning.

You can read more about how results develop over a longer period on our page covering what Wegovy weight loss looks like at six months, which gives a clearer sense of where the trajectory tends to lead. For context on what the first month typically delivers, our month-one Wegovy page sets the baseline.

One practical point worth flagging: because Wegovy pens need to be ordered and prescribed before each new supply, your timing matters more than people expect. If you want a fuller picture of what to expect from Wegovy after 2 months, including how the experience tends to shift as your dose increases, that page is worth a read before your next prescription. A quick call to our support team can usually solve that, but it is worth thinking about in advance.

Side effects that are most active at the two-month mark

The two-month window tends to overlap with the period of most active dose adjustment, which is also when side effects are most frequently reported. Nausea is the most common, followed by constipation, loose stools, burping and reflux. For most people these are mild to moderate and ease within a few days of each dose step. They are worth tolerating if possible, because reducing the dose or pausing treatment restarts the adjustment process.

Fatigue and mild headaches appear for some people at this stage, usually in the first week after moving to a new dose. Staying well hydrated and eating smaller, lower-fat meals helps significantly. The NHS medicines information for semaglutide provides a full side-effect summary, and you should always have the Patient Information Leaflet that comes with your pen to hand.

A small number of people experience more severe abdominal pain. Persistent, severe stomach pain that radiates to the back, with or without vomiting, needs urgent medical attention, this can be a sign of pancreatitis, which is a known but infrequent side effect of GLP-1 medicines. Do not wait and see with that symptom. If you notice any side effect you are unsure about, you can also report it directly via the MHRA's Yellow Card scheme.

Our semaglutide overview covers the mechanism and safety profile in more depth if you want to understand what is happening biologically at each dose.

What to do if two months in you are not sure it is working

The first thing to check is whether your expectations matched the timeline. If your appetite has reduced and you are eating less but the scale is moving slowly, treatment is likely doing what it should, the full weight-loss response comes later. If you have not noticed any appetite change at all and have had no side effects whatsoever, it is worth discussing whether your titration schedule needs reviewing with your prescriber.

For people who reach six months at the highest tolerated dose with less than 5% weight reduction, NICE's guidance suggests reconsidering whether treatment should continue, but that review point is months away at the two-month stage. There is no clinical basis for stopping Wegovy at eight weeks because progress looks slow.
