This is a question our prescribers hear most weeks, and the honest answer is frustrating for many people: standard UK private medical insurance was not designed with weight management medicines in mind. Conditions such as obesity have historically been excluded from PMI (private medical insurance) cover on the grounds that they are considered lifestyle-related rather than acute illnesses requiring intervention. The position has been slow to change, even as clinical evidence mounts that medicines like tirzepatide and semaglutide produce sustained, meaningful weight reduction far beyond anything diet alone can achieve.
Some employer group health schemes (particularly larger corporate schemes administered through providers like Bupa, AXA Health or Vitality) have begun exploring cover for GLP-1 medicines as part of occupational health packages. A small number of top-tier plans now include it as an optional rider or through specialist pathways. If you hold private health insurance through your employer, it is worth reading your policy schedule carefully and asking your HR team or insurer directly, because the landscape is genuinely evolving.
Critical point: even where a policy does cover weight management medicine, the insurer will almost always require a clinical prescription from a registered prescriber. Cover does not mean automatic access — the medicine remains prescription-only regardless of who is funding it, and a prescriber's independent assessment is the starting point.
The NHS does fund tirzepatide for weight management under NICE technology appraisal TA1026, published in December 2024. The current criteria, however, are deliberately phased. As of mid-2026, NHS tirzepatide is available to adults with a BMI of 40 or above who also have four or more of a defined set of weight-related conditions, including type 2 diabetes, high blood pressure, dyslipidaemia, obstructive sleep apnoea and cardiovascular disease. A second cohort covering BMI 35–39.9 with four or more of those conditions activated in June 2026. Further cohorts are expected in 2027. Ethnic-background adjustments apply, with thresholds 2.5 kg/m² lower for some groups.
That leaves a substantial proportion of people who meet the licensed eligibility criteria for private treatment (BMI of 30 or above, or 27 or above with at least one weight-related condition) but who sit outside the current NHS threshold. For them, private prescription is the realistic near-term option. Separately, Wegovy (semaglutide) is available on the NHS through specialist weight management services under NICE TA875, though waiting times in many areas are considerable.
Travel insurance and private health insurance are separate products with separate rules, and the distinction matters if you take weight loss injection medicines on holiday or for extended trips abroad. Standard travel policies typically require you to declare pre-existing medical treatments, which includes any prescription medicine you use regularly. Failing to disclose this could invalidate a claim.
Beyond disclosure, there are practical questions: will the policy cover the cost of replacement medicine if a pen is lost, damaged or stolen abroad? Will it cover treatment needed as a result of a side effect that requires medical attention in another country? The answers vary by insurer and policy tier. Framing this as a simple cost question misses most of what's relevant. The more useful questions are: Is the supply chain verified and regulated? Is a qualified prescriber conducting a real clinical assessment, not a checkbox form? Is there clinical oversight if your health changes or a side effect needs attention?
