Can You Get Weight Loss Injections Free on the NHS?

You've looked it up, found conflicting answers, and you're not sure where you stand

If you've spent twenty minutes reading different websites and still aren't certain whether you can get weight loss injections on the NHS, that's understandable, the rollout is genuinely complicated, and it is changing by cohort every few months. The honest answer is that free access exists, but it is rationed carefully and moving slowly.

NHS England is phasing in tirzepatide (Mounjaro) for weight management under NICE technology appraisal TA1026, published in December 2024. The first cohort, active from June 2025, requires a BMI of 40 or above plus four or more of five specific conditions: high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, or type 2 diabetes. A second cohort covering BMI 35–39.9 with four or more of those conditions was activated on 23 June 2026. Further cohorts will follow, expanding eligibility as NHS capacity grows. Lower BMI thresholds apply for people from South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, typically 2.5 kg/m² below the figures above.

Separately, Wegovy (semaglutide) is available through specialist weight management services under NICE guidance TA875. That route is limited to people already in, or referred to, a multidisciplinary specialist service, and waiting lists in most areas stretch to many months. Why the criteria are so strict right now

NICE and NHS England built the phased rollout around clinical need and NHS capacity. The cost of treatment, the requirement for dietary and activity support alongside prescribing, and the scale of demand all factor into why the early cohorts are restricted to people with the highest clinical complexity.

GP practices can prescribe under the 2026/27 GP contract, but participation is optional, so availability varies by practice even within the same area. Meeting the national criteria does not guarantee your surgery is currently offering the service. NHS England is explicit on this point in its weight management injections guidance: every patient must also be enrolled in wrap-around diet and lifestyle support, which takes capacity to deliver.

For people with a BMI of 35 or above and relevant conditions who are not yet in a qualifying cohort, or whose GP practice has not yet activated the service, the wait is real. That is the situation many people searching this question are actually in, not failing to understand the rules, but genuinely caught between eligibility on paper and access in practice.

What the private route offers while you wait, or if you don't qualify yet

Private prescribing of tirzepatide and semaglutide follows the licensed eligibility in the medicines' SmPCs: adults with a BMI of 30 or above, or 27 or above alongside at least one weight-related condition such as prediabetes, hypertension, high cholesterol, or obstructive sleep apnoea. The clinical bar is lower than the NHS's complex-case criteria, though it is still a clinical bar, a prescriber reviews every case individually.
