What BMI Do You Need for Mounjaro on the NHS?

Under NICE's appraisal of tirzepatide (TA1026), the NHS started prescribing Mounjaro from June 2025 to adults with a BMI at or above 40 who also have four or more of five specific weight-related conditions: high blood pressure, dyslipidaemia (raised cholesterol or triglycerides), obstructive sleep apnoea, established cardiovascular disease, or type 2 diabetes. All five must be present for a patient to count their comorbidities — any four qualifies.

That combination of a very high BMI and multiple conditions puts a strict filter on who gets access first. The threshold exists because the rollout is deliberately staged by clinical need.

One important nuance: if your background is South Asian, Chinese, Middle Eastern, Black African or African-Caribbean, NICE applies the same thresholds but 2.5 kg/m² lower throughout. So the opening cohort threshold for those groups is effectively 37.5, not 40. This reflects evidence that metabolic risk rises at a lower BMI in those populations.

The phased opening, and what 'around June 2026' actually means

NHS England published an interim commissioning plan that staggers access over roughly two years. The second cohort, for adults with a BMI of 35–39.9 who have four or more of the qualifying conditions, was due to open around June 2026. Cohort three, which drops the number-of-conditions requirement to three (while keeping the BMI at 40 or above), is planned for around March 2027.

Two caveats are worth knowing before you plan around those dates. First, GP practices were given the option to prescribe under the new 2026/27 contract from April this year, but participation is voluntary, some surgeries are prescribing, others are not. Second, every NHS patient must take part in a structured diet and activity support programme alongside the medicine; receiving the prescription is not the end of the process, it is the start of a supervised programme.
