What BMI Is Wegovy Approved For — and What Else the Licence Requires

The myth: Wegovy is only for people with a BMI over 35

This one comes up often, and it's understandable, bariatric surgery has historically had a 35-plus threshold attached to it, and some people assume injectable weight-loss medicines sit in the same bracket. They don't. The UK licence for Wegovy (semaglutide 2.4 mg injection) sets the lower band at a BMI of 27, not 35. The 35 figure does appear in the NICE technology appraisal, but that's the NHS commissioning threshold, not the licensed indication. Conflating the two leaves a lot of people thinking they don't qualify when, clinically, they might.

The licensed criteria, as set by the MHRA, are straightforward: a BMI of 30 or above for weight management on its own, or a BMI between 27 and 29.9 alongside at least one weight-related condition. Those conditions include type 2 diabetes, high blood pressure, dyslipidaemia, obstructive sleep apnoea and cardiovascular disease, among others. One practical note on ethnic background: for South Asian, Chinese, other Asian, Middle Eastern, Black African and African-Caribbean adults, UK clinical guidance typically applies thresholds 2.5 kg/m² lower, recognising that cardiovascular and metabolic risk can be elevated at lower BMI readings. What the NICE recommendation adds, and why private treatment is different

NICE's appraisal of semaglutide (TA875, published March 2023) recommends it on the NHS, but with additional restrictions that go beyond the licence itself. NHS access requires treatment within a specialist weight management service with multidisciplinary support, a BMI of at least 35 (or 32.5 for the ethnic-background groups above), and at least one weight-related comorbidity. Critically, NICE caps NHS use at a maximum of two years. These are commissioning conditions, they determine who the NHS will fund, not who the medicine is clinically suitable for.

Private treatment through a regulated pharmacy operates against the licensed criteria, not the NICE commissioning criteria. That means the BMI thresholds are lower (30 on its own; 27 with a condition), there is no mandatory specialist-service referral, and there is no two-year hard stop written into the licence. What a prescriber looks at beyond the BMI number

BMI is a starting filter, not a verdict. A prescriber reviewing a Wegovy consultation will also consider your current medications (some interact with semaglutide's GI profile), your personal and family history (including any history of pancreatitis or medullary thyroid carcinoma, which are contraindications), and whether you're pregnant, breastfeeding or trying to conceive, Wegovy is not recommended in any of those situations.
