What does the Wegovy needle look like — and what happens when you inject?

Step 1: what you're looking at before you even open the cap

Pull the Wegovy pen out of the fridge and you'll see a device roughly the length and weight of a thick marker pen. The body is colour-coded by dose (the shade changes at each strength in the titration schedule), and a window on the side lets you see the liquid inside. The needle itself is completely hidden at this stage. It sits inside the base cap, which has a yellow inner needle shield directly over the tip. Nothing sharp is exposed. Step 2: removing the cap and seeing (briefly) the needle shield

Twist and pull the cap off with a firm motion. What you see is not the needle itself but the inner needle shield: a small pale-coloured cone or tube that sits flush over the tip. The needle is inside that shield and still not visible. This is the correct appearance; if you see a bare metal tip at this point, the shield has failed and you should not use that pen.

The shield is spring-loaded. Press the pen firmly against your skin and it retracts, allowing the needle to enter the subcutaneous layer, typically 4–5 mm deep, which reaches just beneath the skin surface without touching muscle. This short length is enough for subcutaneous delivery and is one reason the injection is far less uncomfortable than most people expect.

Once you've picked your site (abdomen, outer thigh or upper arm), clean the area, then press and hold the pen against the skin for about 10 seconds after pressing the dose button. Step 3: what the pen looks like after you've injected

Lift the pen away and the needle shield springs back out automatically, locking permanently over the tip. You cannot retract it again. This is the pen telling you it has been used. The indicator window will have moved to show the dose has been delivered. If you're unsure whether the full dose went in (perhaps the button didn't travel completely or you lifted the pen early) do not inject again without speaking to your prescriber first; double-dosing to compensate is not the right step.

The used pen, needle locked inside, goes straight into a dedicated sharps container for safe disposal. Never put a used pen in household recycling or a standard bin. A note on needle changes and the higher 7.2 mg pen format

One question that comes up with pen injectors generally: do you need to change the needle between doses? For Wegovy, the answer is no, each pen is pre-attached and single-use per dose session; there is no user-replaceable needle component in the standard format.
