Semaglutide slows the rate at which food leaves the stomach. That is partly how it reduces appetite, you feel full for longer. The catch is that fat already slows gastric emptying on its own, so a greasy meal on top of Wegovy creates a kind of compound delay. The stomach stays full far longer than your body expects, and the result is often pronounced nausea, bloating or, in some cases, vomiting.

Fried foods are the most reported trigger in clinical experience. Takeaway chips, battered fish, pastries cooked in deep fat and heavily buttered sauces all fall into this category. It is not that a small amount causes irreversible harm, it is that the gastrointestinal system is much less forgiving while it adapts to treatment, and a rich meal can set off discomfort that lasts hours. Cutting right back on saturated fat (not just fried food but fatty cuts of meat, full-fat processed cheeses and creamy sauces) tends to be one of the most effective things people can do to get through the adjustment period more comfortably. Leaner proteins and smaller, more frequent meals are generally better tolerated, though the specifics are worth discussing with your prescriber.

What can you not eat on Wegovy when it comes to sugar and refined carbohydrates?

Sugary drinks (fizzy drinks, fruit juice, energy drinks, even some "wellness" smoothies) deliver a rapid glucose load that can cause blood-sugar fluctuations and worsen nausea. Because semaglutide affects the way insulin and glucagon respond to meals, these swings feel more pronounced for some people.

Ultra-processed foods with high sugar content (biscuits, confectionery, sweet cereals) are also worth avoiding, though for a different reason: they tend to be calorie-dense and nutritionally thin, which works directly against the treatment's purpose. Wegovy reduces appetite, but if what you do eat is mostly refined sugar and little protein or fibre, muscle loss becomes a real concern over months of treatment.

Refined carbohydrates more broadly (white bread, white rice in large portions, pastries) are not forbidden, but they lack the fibre and protein that help steady blood sugar and keep you satisfied between meals. Does alcohol interact with Wegovy in ways that matter?

Yes, and it is one of the less-discussed points. Alcohol on its own can cause blood-sugar drops, particularly if someone has not eaten much, and reduced appetite is precisely what Wegovy produces. The combination increases the likelihood of hypoglycaemia-like symptoms: dizziness, sweating, shakiness. This matters most for people who also have type 2 diabetes or prediabetes, but it applies more broadly too.

Beyond blood sugar, alcohol is a gastric irritant. The stomach and gut are already adjusting to semaglutide's effects on motility and emptying. Adding alcohol tends to worsen nausea and can trigger vomiting more readily than it would off treatment. Some people also find that their tolerance for alcohol changes noticeably on Wegovy, a drink that felt manageable before can have a stronger effect.

There is no licensed guidance that says alcohol is absolutely prohibited, but most clinical advice is to keep intake low and to eat something before drinking. Are there any other foods that are worth being cautious about?

Spicy food is not universally problematic, but people with existing reflux or indigestion find that Wegovy can make heartburn more likely, and spicy dishes add to that. The lower oesophageal sphincter is affected by changes in gastric pressure, a stomach that empties slowly creates more opportunity for acid to travel upward. If you are prone to reflux, it is worth being cautious with very hot curries, chilli sauces and similar foods, particularly in the first few months.

Large portions generally are worth avoiding, regardless of what is in them. Because gastric emptying slows, eating the same volume you managed before treatment is likely to cause discomfort, a sensation of excessive fullness or pressure that can last hours.
