The refrigerator rule and why it exists

Mounjaro is a biological medicine. The active molecule, tirzepatide, is sensitive to temperature in a way that most tablets simply are not. At 2°C to 8°C, it remains stable for the duration shown on the packaging. Take it outside that band and you risk protein degradation that is invisible to the eye, the liquid can look perfectly clear while no longer working as it should.

In practice, the fridge at home is the right place. The middle shelf tends to offer the most consistent temperature; the fridge door, where it warms every time you reach for the milk, is less ideal. Keep the pen in its original cardboard sleeve: Mounjaro is light-sensitive, and exposure to UV accelerates the same degradation that heat causes. The room-temperature window, what the SmPC actually says

The Mounjaro SmPC does permit a finite period of room-temperature storage, which is relevant when you are travelling, at work, or somewhere your pen cannot be refrigerated. The precise number of days and the maximum temperature ceiling for that window are set out in the Mounjaro SmPC on the electronic Medicines Compendium (eMC) and in the Patient Information Leaflet inside your pen box.

The reason this guide does not state the exact figure here is deliberate. The licensed Mounjaro room-temperature window is defined for a specific maximum ambient temperature, and that ceiling matters as much as the duration. A warm summer flat, a bag left in a sun-facing car, or a holiday destination with higher ambient temperatures can push conditions beyond what the label permits faster than you might expect. Reading the leaflet for your own pen is the only way to apply the rule correctly to your situation. If your pen has been out of the fridge and you are unsure whether it remained within the permitted conditions, your pharmacist or prescriber is the right person to ask before you inject.

Situations that call for an immediate decision

Most storage problems fall into two categories: accidental cold exposure (a pen pushed to the back of a fridge that has iced over) and accidental heat exposure (a pen left in a gym bag in a hot car all afternoon). Both require the same first step: do not guess and inject anyway.

A pen that has been frozen should be treated as unusable. One that has been in heat or direct sunlight for an uncertain period is also suspect. In either case, contacting your prescriber or pharmacist is the right call. Practical storage and disposal once the pen is in use

Each Mounjaro KwikPen contains four weekly doses. Once you have used the pen for the first time, return it to the fridge between doses. The same 2°C to 8°C rule applies throughout its in-use period until the final dose is administered.

Used needles are clinical waste. A 1-litre sharps container keeps used needles safely contained and can be returned to a participating pharmacy for disposal. Never place used needles in domestic recycling or general waste.
