Imagine sitting down to a meal three weeks into treatment and realising you have eaten half your usual portion and feel done. That is Wegovy working. Semaglutide slows gastric emptying and signals fullness through GLP-1 receptors, which means total food intake drops. The risk is not overeating, it is under-nourishing.
The NHS healthy weight guidance recommends building meals around vegetables, lean protein and wholegrains. On Wegovy, protein becomes the priority: it supports muscle retention during weight loss and keeps hunger settled between meals. Aim for a palm-sized portion of chicken, fish, eggs, legumes or dairy at each meal, even if the portion overall is smaller than before. Fibre helps too (vegetables, oats, lentils) partly for satiety, partly for gut health, which matters when the medication is already slowing digestion.
Hydration is underrated here. Reduced appetite can reduce thirst signals as well, and dehydration tends to amplify the nausea some people notice at the start of treatment or after a dose step. Keep water accessible throughout the day. Alcohol is not prohibited but sits alongside fatty or rich food as something many people on semaglutide find their tolerance for has shifted, worth noting before a celebration.
If you find yourself eating very little for several days, contact your prescriber. Severe restriction on top of a GLP-1 medicine is not the goal, and your clinical team can help you find the balance.
Here is something that surprises people: not all exercise produces the same result when you are losing weight with a GLP-1 medicine. Cardiovascular activity is good for heart health and mood, and it counts. But when weight loss is rapid, the body can reduce muscle alongside fat, and muscle is metabolically active tissue you want to keep.
Resistance exercise (bodyweight movements, resistance bands, weights at a gym, or even heavy gardening) signals to the body to preserve muscle. It does not need to be intense. Two or three sessions a week of 20 to 30 minutes is a reasonable starting point, built up gradually. If your joints are managing and your energy allows, adding more is fine.
A question our prescribers hear regularly: what if I am too tired in the first few weeks to do much? That is common. Nausea, fatigue and adjustment are normal in the early dose period. Gentle walking is enough while you settle. The important thing is that activity increases over time, not that it is heroic from week one.
The NHS England guidance on weight-management injections is clear that lifestyle changes alongside medicine (rather than medicine alone) produce the best results. Wegovy is a tool, not a replacement for movement.
Most side effects from Wegovy are gastrointestinal (nausea, loose stools, constipation, indigestion, burping) and they tend to be most noticeable in the first days after starting or stepping up a dose. For many people they settle within a week or two. Eating bland, lower-fat meals and avoiding large portions helps during those early days. Some symptoms need prompt attention. In January 2026 the MHRA issued a Drug Safety Update reminding patients and clinicians that acute pancreatitis (though uncommon) can occur with GLP-1 medicines: severe, persistent stomach pain that spreads to the back, with or without vomiting, means stopping the injection and getting urgent medical help. Do not wait to see if it settles. Signs of a serious allergic reaction, gallbladder pain, or significant dehydration from persistent vomiting or diarrhoea are also reasons to seek care without delay.
If you are taking any oral medicines, including the contraceptive pill, tell your prescriber, GLP-1 medicines can affect absorption of some oral drugs. Any suspected side effect can be reported directly to the MHRA at yellowcard.mhra.gov.uk. You do not need to wait for a GP appointment to do so.

Wegovy is a once-weekly injection, taken on the same day each week where possible. Life gets in the way, that is realistic. If your chosen day shifts by a day or two, it is generally fine; the Patient Information Leaflet gives the specific guidance on missed doses, and your prescriber can advise on your individual situation. What matters is consistency over months, not perfection on every single date.
Repeat orders are worth thinking about a little ahead of time. If your delivery lands on a Tuesday and you order the week before, you are fine. If you are planning a holiday, ordering before you travel avoids the stress of managing a gap.
