Wegovy starts at 0.25mg weekly. That opening dose is a tolerability step: its job is to let your system adjust to the medicine, not to deliver immediate results. Some people feel a noticeable drop in hunger within the first week or two; others find the effect subtle at first. Either is normal.
Nausea is the most commonly reported early experience, particularly in the days following each injection. It tends to be mild-to-moderate and often passes within a few days. Keeping meals smaller, eating slowly and avoiding very fatty or rich foods makes a real difference. Some people find keeping the pen in the same spot in the fridge door (next to where they keep the milk) helps build the routine of weekly injecting at the same time each week.

One thing worth knowing early: Wegovy is a prescription medicine and every step of treatment, including dose increases, is reviewed by a prescriber. There are no automatic changes.
From 0.25mg, the schedule moves through 0.5mg, then 1.0mg, each held for roughly four weeks. Most people begin to notice more consistent appetite suppression from around the 0.5mg mark. Portions feel satisfying sooner. Thinking about food between meals becomes less constant. For many, this is where the experience of Wegovy starts to feel meaningfully different from trying to lose weight without support.
Gastrointestinal side effects can resurface briefly after each dose step up (nausea, loose stools or constipation are most common) before settling again. The pattern tends to repeat with each increase, but most people find the episodes shorter and milder at higher steps because the body has already adapted to the mechanism.
Weight loss typically accelerates through this phase. There is no single trajectory that applies to everyone: age, starting weight, diet, activity levels and individual response to the medicine all shape the result. The standard maintenance dose for Wegovy is 2.4mg weekly, reached after around 16 weeks of titration. This is where the STEP 1 trial's results were generated: roughly 15% average weight reduction over 68 weeks, in a study of nearly 2,000 adults with obesity or overweight and a weight-related condition. That figure is a population average, some participants lost considerably more, some less. It gives an honest reference point, not a personal guarantee.
The MHRA also approved a 7.2mg maintenance dose in January 2026, and in April 2026 a dedicated single-dose 7.2mg pen was confirmed, with trials reporting around 20.7% average weight loss at 72 weeks. Whether that higher dose is clinically appropriate for a given patient is a decision made with a prescriber, not a default upgrade. At maintenance, the focus shifts: sustaining weight loss requires continuing the medicine alongside realistic eating habits and regular activity. Weight tends to return if treatment stops without those foundations in place.
