The NHS route: who now qualifies and what to expect

Mounjaro became available on the NHS following NICE's recommendation in December 2024 (updated September 2025). Access is being phased in gradually. From June 2025, the NHS began prescribing it to adults with a BMI of 40 or above who also have four or more of five specific weight-related conditions: high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, or type 2 diabetes. A second cohort (BMI 35–39.9 with four or more of those conditions) became eligible in June 2026.

From April 2026, GP practices that have signed up to the relevant NHS contract can prescribe Mounjaro directly, though not every practice has opted in yet. If yours hasn't, your GP may refer you to a specialist weight management service instead. Every NHS patient is also expected to take part in wraparound dietary and activity support alongside the medicine, as set out by NHS England's guidance on weight management injections.

Lower BMI thresholds (2.5 kg/m² below the standard figures) apply for people from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds. Meeting the criteria still doesn't guarantee immediate access; waiting times vary considerably by area and practice.

The private route: what legitimate looks like

Red flags: why the source of your medicine matters

The counterfeit problem around GLP-1 medicines in the UK is serious and documented. The MHRA has warned publicly about fake weight-loss pens sold via social media and unregistered websites, some seized pens have contained insulin rather than tirzepatide, leading to hospitalisations.
