Where to inject Wegovy in your leg — the right spot and how to find it

The target zone is the outer, middle third of the thigh, not the inner thigh, not the front (quadriceps), and not too close to the knee or groin.

The outer thigh is the correct site when injecting Wegovy into the leg. How to locate, prepare and use the thigh site safely, a step-by-step guide

Step 1, Finding the right part of the thigh

Sit in a comfortable chair with your leg resting flat, slightly relaxed. Look at the outer face of your thigh, not the front face where the quadriceps muscle sits, and not the inner thigh, which has a richer network of blood vessels and nerves running close to the surface.

Picture the thigh divided into thirds from knee to hip. The middle third on the outer side is your target. In practice this means a hand's width above the knee and a hand's width below the hip, on the outside of the leg. If you run your hand along the outer seam of your trousers, the injection zone broadly follows that line in the middle section.

Avoid areas that feel hard or have visible lumps. If you have scarring from previous injections, move at least 1–2 cm away from it. That tissue absorbs medicine less reliably, and the same goes for bruised skin. Step 2, Preparing the site and the pen

Wash your hands thoroughly with soap and water first. Then, if you have them, wipe the skin with an alcohol swab and let it dry completely, injecting through wet skin can sting more and may affect the site. The pen itself should have come out of the fridge at least 30 minutes before you inject; a pen at room temperature is noticeably more comfortable than a cold one.

Your Wegovy pen arrives via tracked DPD delivery in plain, unmarked packaging. If it has been in transit, check the pen is clear and colourless before use and has not been frozen at any point. Remove the pen cap just before you inject. Do not prime the pen by firing a test shot, Wegovy pens are pre-set and single-use; pressing the button outside the skin wastes a dose. Check that the expiry date has not passed and that the liquid in the viewing window looks right.

Step 3, Injecting into the thigh correctly

Pinch around 5 cm of skin between your thumb and two fingers to lift a fold of subcutaneous tissue away from the muscle underneath. This is especially useful for leaner thighs where the fat layer is thinner. Position the pen at a 90-degree angle to the pinched skin, straight in, not at a slant. Place the pen firmly against the skin until the yellow safety cap retracts, then press and hold the button until you hear or feel the click and the yellow indicator confirms the dose is complete.

Hold the pen in place for about six seconds after the click before withdrawing it in one smooth motion. Release the pinched skin, then apply gentle pressure with a clean fingertip or cotton pad if there is any minor bleeding. Do not rub, rubbing can irritate the tissue. Dispose of the used pen in an approved sharps bin; used pens should never go into household recycling or general waste.

Rotating sites and common issues with the thigh

Rotation matters more than many people expect. Injecting in the same small spot week after week creates lipohypertrophy, fatty lumps under the skin that feel firm and absorb semaglutide unpredictably. This can make your doses less effective without any obvious reason. A simple rotation pattern might be: left thigh one week, abdomen the next, right thigh the week after, then the upper arm, and so on.

The most common local reactions at the thigh site are mild redness, a small raised weal or bruising, all of which normally fade within a couple of days. If you notice persistent swelling, hardening, or skin changes at a previous injection site, mention it to your prescriber before the next dose.
