Does the NHS offer weight loss injections, and who qualifies?

Yes, but access is tightly controlled and phased in over several years. Mounjaro (tirzepatide) was recommended by NICE in December 2024 (Technology Appraisal TA1026) for adults with a BMI of 35 or above and at least one weight-related condition such as high blood pressure, type 2 diabetes, dyslipidaemia, obstructive sleep apnoea or cardiovascular disease. The current NHS rollout gives priority to those with a BMI of 40-plus and four or more of those conditions; a second cohort covering BMI 35–39.9 with four or more conditions opened in June 2026. Further cohorts will follow into 2027.

Wegovy (semaglutide) is also available on the NHS but only through specialist weight management services, and waiting lists are commonly long. NHS access requires participation in a structured diet and activity programme alongside the injection. Even when someone meets the criteria on paper, their GP practice or local integrated care board has to be participating, and not all are. For those who meet neither the current NHS cohort criteria nor the specialist-service route, private treatment is the practical alternative. It is not a workaround, it is the same licensed medicine prescribed lawfully through a regulated pathway.

Which private providers can legally prescribe weight loss injections?

Private providers fall into three broad categories: high-street private clinics, private GP services and regulated online pharmacies. A face-to-face appointment is not a legal requirement, but the clinical assessment is.

Online pharmacies have become the most commonly used private route because they remove the need for travel and appointment slots. The key check is whether the pharmacy appears on the GPhC register, every UK online pharmacy selling medicines must be listed there. If it is not on the register, do not use it. That single check filters out the vast majority of illegitimate sellers.

What red flags tell you a provider is not legitimate?

This question matters because fake weight loss pens are a genuine public health problem. The MHRA has conducted large enforcement operations (seizing around 20 million doses of illegally traded weight loss medicines (approximately £45 million in street value)) and the first UK site manufacturing counterfeit tirzepatide pens was raided in late 2025. Some fake pens have contained insulin, causing serious hypoglycaemic episodes in people who had no idea what they were injecting. The MHRA Yellow Card scheme allows anyone to report a suspect seller or a counterfeit product.

The clearest red flags: a seller offering these medicines without asking you to complete a medical questionnaire or speak to a prescriber; prices significantly below the market range; sellers operating only through social media or messaging apps; and any promotion of "discount codes" for prescription medicines. Storage is also a useful real-world check. Genuine Mounjaro and Wegovy pens must be kept refrigerated at 2–8°C until use, a legitimate provider will tell you this clearly (your pen belongs in the fridge, not at the back of a warm courier depot for a fortnight). If a supplier never mentions cold-chain delivery, that is worth questioning.

How does the clinical assessment work at a regulated private pharmacy?

The process at any properly run provider follows the same logical sequence: you complete a detailed medical questionnaire, a prescriber reviews it and decides whether treatment is clinically appropriate, and the prescription is only issued if the assessment is passed.
