Can an NHS GP prescribe Mounjaro for weight loss right now?

Yes, in principle. From 1 April 2026, GP practices in England gained the ability to prescribe tirzepatide (Mounjaro) for weight management under the new QOF contract. In practice, participation is voluntary, so not every surgery offers it. If yours does, you still need to meet current NHS eligibility: a BMI of 35 or above plus four or more of five specific weight-related conditions, high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, or type 2 diabetes. The thresholds are 2.5 kg/m² lower for some ethnic backgrounds under NICE's appraisal of tirzepatide (TA1026).

Cohort 2, which opened in June 2026, extends access to people with a BMI of 35 to 39.9 alongside four or more conditions. A further expansion to three qualifying conditions is planned for around March 2027. The criteria are strict by design; the programme is phased because demand is enormous. If your BMI is, say, 31 with one or two conditions, the NHS pathway is not currently available to you regardless of how willing your GP is. Every NHS patient also has to engage with wrap-around diet and activity support as part of the programme. Meeting the numbers on paper does not guarantee approval, your GP will assess your full picture before issuing any prescription.

What if my GP's surgery is not taking part?

This is where a lot of people get stuck. Your GP may be entirely willing but their practice has not yet opted in, or local commissioning has not activated the programme. That is not a reflection on you or on tirzepatide's evidence base, it is an administrative reality of a rolling national programme. If your surgery is not participating, you have two options: wait, or explore a private prescription. A private prescriber can assess and prescribe independently of any NHS programme, including for people who do not meet NHS thresholds. The licensed eligibility for a private prescription is broader: adults with a BMI of 30 or above, or 27 or above with at least one weight-related condition such as prediabetes, hypertension or high cholesterol. A prescriber still assesses your whole health picture, BMI alone is not the only consideration.

Does cost affect whether a GP will prescribe it?

It can. On the NHS, prescribing decisions are subject to local commissioning budgets as well as clinical criteria. GPs in practices that have joined the programme work within guidance on eligible cohorts partly because of the medicines budget implications. Eli Lilly raised Mounjaro's UK list price significantly from 1 September 2025; as widely reported, the highest-dose pen's list price moved from around £122 to around £330 for a four-week supply. That has implications for NHS funding allocations, and it feeds into the phased rollout design.
