Start journey Learn moreSemaglutide does not directly boost energy in the way a stimulant might, but many people report feeling more alert and physically capable as their weight falls. Clinical trial data suggest the fatigue some people notice early in treatment tends to ease, and the longer-term picture is more encouraging. Wegovy is a prescription-only medicine, and whether it suits you is a question a prescriber assesses individually. This page unpacks what the evidence says about energy levels during treatment — and what to do if tiredness is getting in the way.

The landmark STEP 1 trial, published in the New England Journal of Medicine, tracked adults with obesity over 68 weeks on semaglutide 2.4mg. The headline result was an average body-weight reduction of around 15%, but quality-of-life scores also improved significantly by the end of the study. Participants reported better physical functioning, the kind of difference that shows up as climbing stairs without pausing or keeping up with the school run.
Fatigue did appear in the side-effect data, but it was concentrated in the early weeks. It ranked lower than nausea in frequency and, for most participants, faded rather than persisted. The mechanism is worth understanding: semaglutide slows gastric emptying and reduces appetite sharply. When calorie intake drops quickly, the body needs a week or two to recalibrate. That adjustment period is real, but it is not the whole story of the treatment.
The longer-term picture, once weight starts to fall meaningfully, points in the opposite direction. Less body mass to carry, a lighter load on the heart and lungs, and reduced joint pressure all feed back into how energetic daily life feels. If you are wondering whether Wegovy gives you energy, the honest answer is that this is not a direct pharmacological effect of semaglutide, it is the downstream consequence of sustained weight loss, and it is why the trial's quality-of-life scores kept improving across the full 68 weeks.
If you are a few weeks into treatment and feeling sluggish rather than energised, you are not imagining it, and you are not alone. Our prescribers hear this regularly. There are a few overlapping reasons it happens.
The most common is reduced food intake without a compensating shift in what is eaten. Semaglutide suppresses appetite substantially. If the calories that disappear were mostly from refined carbohydrates, blood-sugar rhythms can flatten in a way that feels like a steady low-grade tiredness. Protein becomes proportionally more important, it supports muscle and keeps satiety steady between meals. Dehydration is a related issue: nausea can make it harder to drink enough, and even mild dehydration compounds fatigue.
Dose increases are another trigger. The titration schedule for Wegovy, which runs from a low starting dose upward in roughly monthly steps under prescriber guidance, can bring a short wave of GI symptoms and tiredness each time the dose steps up. This is why the titration exists, to give the body time to adapt rather than overwhelming it. The NHS semaglutide page notes fatigue as a known side effect and advises speaking to your healthcare team if it is persistent.
Separate from semaglutide itself: people who carry a significant amount of weight often have disrupted sleep, from obstructive sleep apnoea, joint discomfort, or simply the metabolic burden of obesity. As weight falls, sleep can improve, but that shift takes time and may not be linear.
Most tiredness on Wegovy resolves within two to four weeks of a new dose. Persistent exhaustion that does not ease, or fatigue accompanied by dizziness, rapid heartbeat, or significant weakness, is worth reporting to your prescriber rather than waiting out. These could point to inadequate nutrition, dehydration, or (less commonly) another clinical issue that warrants investigation.
The MHRA's guidance on GLP-1 medicines highlights that serious side effects are rare, but patients should report unexpected or severe symptoms promptly. The Yellow Card scheme at yellowcard.mhra.gov.uk allows anyone to report a suspected side effect directly to the regulator.
One practical note: people often feel they should push through fatigue on treatment because weight loss is happening. That instinct is understandable, but rest and adequate nutrition are not in conflict with the treatment working. Strength-based activity, even light resistance work, helps preserve muscle mass during weight loss, and many people find that building more energy on Wegovy comes partly from pairing the treatment with consistent movement and good nutrition, rather than resting on the medication alone. Details of the right approach for you sit with your prescriber, not a general content page.
If you want to read more about people's experiences with tiredness specifically, the page on managing low energy on Wegovy covers practical steps, and our overview of how semaglutide affects energy levels goes deeper into the pharmacology.
By around six months of treatment, the pattern most people describe is materially different from the early weeks. The appetite suppression feels familiar rather than disorienting, the GI side effects have settled, and the physical effects of reduced body weight start to compound. Less load on the cardiovascular system means lower resting heart rate and less breathlessness. Joints under less pressure allow longer, easier movement. Sleep, as noted, often improves.
None of this is guaranteed, individual responses vary, and some people find energy remains variable throughout treatment. But the direction of travel in the trial data is clear: sustained weight loss in people with obesity is associated with improved physical function and self-reported vitality.
Wegovy is one semaglutide-based treatment option. If you are weighing it against other approaches, the page on how Wegovy works covers the broader picture, and our general weight-loss treatment overview explains what regulated private treatment involves. For those curious about how Wegovy compares to other semaglutide formulations, our semaglutide guide covers both the injection and oral options.
