Would I qualify for Mounjaro? Here's what the evidence and licensing actually say

The BMI thresholds come from the licence (and they have some important nuances

NICE's appraisal of tirzepatide (TA1026, published December 2024) set NHS eligibility at BMI 35 or above with at least one weight-related comorbidity) stricter than the broader private licence. For private prescribing, the UK-licensed thresholds (BMI 30 or above, or 27 or above with a relevant condition) remain the operative framework, and those are the thresholds our prescribers work from.

It is also worth knowing that the 27-plus route exists precisely because excess weight causes measurable harm at lower BMIs in many people. Qualifying conditions under this route include prediabetes, established type 2 diabetes, high blood pressure, raised cholesterol, obstructive sleep apnoea, and certain other metabolic conditions. Why your health history matters as much as your BMI score

A BMI calculator is a useful starting point, but a prescriber's assessment goes considerably further. Mounjaro's Summary of Product Characteristics lists several conditions that require careful clinical evaluation before treatment begins. A personal or family history of medullary thyroid carcinoma, or a condition called MEN2 (multiple endocrine neoplasia type 2), means tirzepatide should not be used. A history of pancreatitis warrants a detailed conversation: in January 2026 the MHRA issued a Drug Safety Update specifically noting that acute pancreatitis is a known, if infrequent, serious risk with GLP-1 medicines, and prescribers take that seriously at the assessment stage.

Gallbladder disease, significant kidney problems, and certain gastrointestinal conditions are also part of what a thorough consultation explores. Age and reproductive status also feed into eligibility. Mounjaro is not licensed for anyone under 18. It is not recommended during pregnancy, while breastfeeding, or for anyone actively trying to conceive. Women taking the oral contraceptive pill should know that tirzepatide can reduce pill absorption during the first four weeks of treatment and after each dose increase, meaning a non-oral method such as condoms should be used during those windows, something a prescriber will flag.

NHS versus private: the same medicine, different access routes
