The misconception: 'My GP can just prescribe Mounjaro now'

Since April 2026, the headline has been true in principle. Under the 2026/27 GP contract, practices in England gained the ability to prescribe tirzepatide for weight management through a new QOF framework. That sounds like a green light. In practice, participation is entirely optional, and NHS England's own guidance makes clear that not every practice will take part, at least not yet. Some have no trained staff, no capacity, or no local commissioning support in place.

There is also a second filter: even at a practice that does prescribe Mounjaro, the NHS eligibility criteria are strict. The current cohort (active from June 2025) requires a BMI of 40 or above, plus four or more of five defined conditions: high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, or type 2 diabetes. Every NHS patient must also take part in structured wrap-around diet and activity support, meeting the criteria on paper is necessary but not sufficient. Supply is limited, and waiting times vary significantly by area. When GP prescribing becomes more widely available is a question with an honest answer: for most people, not immediately.

What 'prescription-only' actually means for you

The private route: a GPhC-registered prescriber, not a GP referral
