Can you be prescribed Mounjaro on the NHS — and what if you can't?

You've checked your BMI and think you might qualify, here's what the NHS actually requires

Picture this: you've read that Mounjaro is now available on the NHS, you've worked out your BMI, and you're wondering whether your GP can refer you today. The honest answer is that it depends on exactly which cohort you fall into, and whether your practice is participating at all.

NICE recommended tirzepatide for NHS use in technology appraisal TA1026, published December 2024. The rollout is phased. From June 2025, adults with a BMI of 40 or above and four or more of five specific conditions (high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease or type 2 diabetes) became eligible. The second cohort, covering BMI 35–39.9 with four or more of those same conditions, activated on 23 June 2026. Thresholds are 2.5 kg/m² lower for some ethnic backgrounds under UK guidance.

From April 2026, GP practices can prescribe within the new QOF framework, but the contract is opt-in. A quick way to get a sense of where you stand: check with your surgery whether they are taking part before you build expectations around a referral. That single call can save weeks of uncertainty. What happens when the NHS criteria don't fit your situation right now

Most people asking this question find that they're either in a later rollout cohort or at a practice not yet signed up. That's not a niche problem, the phased structure means the majority of adults who are clinically suitable for Mounjaro will wait months or years for NHS access. Cohort 3, covering BMI 40 with three conditions, isn't expected until around March 2027.

Can your GP prescribe Mounjaro privately, and what does NHS vs private actually cost?

Your GP can prescribe Mounjaro privately if they choose to (there is no rule stopping them) but many won't because private prescribing sits outside their NHS contract and carries its own administrative requirements. On cost: NHS prescriptions carry the standard charge (or are free if you're exempt), but reaching that point requires meeting the phased criteria and accessing a participating service. Since Eli Lilly raised the UK list price in September 2025, typical private market prices have ranged from roughly £149 to £375 per month depending on the strength and the service. The thing worth understanding is that for a prescription medicine, the right question isn't which price is lowest. It's whether the supply chain is legitimate, the prescriber is registered, and the pharmacy is verifiable.
