Wegovy's UK Summary of Product Characteristics (the authoritative document that lists confirmed side effects) does not include anxiety in the common (affecting more than 1 in 100 people) or uncommon categories. The most frequently reported effects are gastrointestinal: nausea, diarrhoea, vomiting and constipation sit at the top of the list. That said, absence from the SmPC doesn't mean nobody experiences it. Spontaneous reports to regulators (the Yellow Card system in the UK) include mood-related entries for GLP-1 medicines as a class. The MHRA and the European Medicines Agency both reviewed this signal in 2023 and 2024 following reports from users and healthcare professionals. Their conclusion, published through the EMA's review in 2024, was that available data did not establish a causal link between semaglutide and anxiety or suicidal ideation, but that monitoring should continue. The situation is still being watched.
So where does that leave you? The honest answer is that a subset of people on Wegovy does report heightened anxiety, the cause isn't pinned down, and regulators are taking it seriously without yet classifying it as an established effect.
Several Wegovy side effects can produce symptoms that feel a lot like anxiety, even if they aren't anxiety in the clinical sense. Nausea is persistent for many people in the first four to eight weeks, and chronic nausea is genuinely exhausting — it raises heart rate slightly, disrupts sleep and creates a background sense of physical unease that the mind can interpret as dread. Poor sleep, which some users report, is itself a reliable trigger for anxious thinking.
Reduced appetite is another factor. People who eat significantly less than usual sometimes experience lightheadedness or a drop in energy, which can feel unsettling. If you take other medicines (particularly anything that affects blood sugar) fluctuations can also produce shakiness and racing thoughts. None of this is Wegovy acting on mood receptors; it's the body adjusting to a significant change in how it processes food and hunger.
Caffeine sensitivity is worth a mention too. Some users find that coffee or tea hits harder on an empty stomach when appetite is suppressed, the kind of jitteriness that follows a strong flat white before you've eaten anything can feel indistinguishable from anxiety for some people. Worth noting if your mornings have felt more fraught since starting treatment.

The threshold for contacting a prescriber about mood changes should be low. This is not a side effect to manage with a self-help podcast and hope. Specific situations that warrant prompt contact:
New anxiety that started within days or weeks of beginning Wegovy or after a dose increase. Low mood that persists beyond two weeks. Any thoughts of self-harm, however fleeting. Panic attacks that are new or substantially worse than before. Anxiety significant enough to affect sleep, work or relationships.
You can also report your experience directly through the MHRA's Yellow Card scheme, patient reports genuinely contribute to the ongoing safety surveillance for newer medicines. Wegovy carries a Black Triangle (▼) status, meaning it is subject to additional MHRA monitoring precisely because it is relatively new.
Do not adjust or stop your dose without speaking to your prescriber first. Stopping a GLP-1 medicine abruptly doesn't cause harm in the way some medicines do, but your prescriber needs to know what's happening and may want to review your dose schedule or rule out other causes. This is a genuinely interesting area of emerging research. Some trial participants reported improvements in quality of life and psychological wellbeing over the course of semaglutide treatment, and there is growing academic interest in whether GLP-1 receptor agonists have independent effects on reward pathways in the brain. The STEP programme trials included quality-of-life measures, and results were generally positive, though that reflects composite wellbeing scores, not anxiety specifically.
A plausible explanation is indirect: losing a meaningful amount of weight, gaining energy, and feeling more in control of eating often improves mood in people who have struggled with weight for years. Reduced joint pain, better sleep apnoea, and more physical capability all contribute to psychological wellbeing in ways that are hard to separate from any direct pharmacological effect.
