Wegovy and Anxiety: What We Know, What's Less Certain, and When to Act

Anxiety is not a commonly reported side effect of Wegovy in UK clinical trial data, but mood changes have been noted in some patients and are monitored by the MHRA under the medicine's Black Triangle (▼) status.
The most frequently reported side effects of semaglutide are gastrointestinal — nausea, diarrhoea, constipation — and these physical symptoms can themselves provoke or worsen feelings of anxiety in some people.
The MHRA's Yellow Card scheme allows patients to report any suspected side effect, including mood changes, directly; this reporting helps regulators spot patterns early.
If anxiety, low mood, or thoughts of self-harm emerge or worsen during Wegovy treatment, contact your prescriber or clinical team the same day, do not wait for your next scheduled review.

Anxiety is not listed among Wegovy's common side effects in its UK prescribing information, but some people starting semaglutide do notice changes in mood or a heightened sense of unease, particularly in the early weeks of treatment. Here is what the evidence says, what the MHRA monitors, and what to do if you're experiencing this. Wegovy is a prescription-only medicine; a prescriber reviews every patient individually before and during treatment, and any mental health concerns should be raised with your clinical team promptly.

The evidence on semaglutide and mood: a step-by-step look at what actually happens

Step 1: Understand what Wegovy's prescribing information does (and does not) say about anxiety

Wegovy's UK summary of product characteristics, which governs how the medicine is prescribed here, does not list anxiety as a common or very common side effect. The dominant side-effect profile across the STEP clinical trials (the programme that supported Wegovy's UK licence) was gastrointestinal: nausea and vomiting in particular, especially in the first weeks of each dose increase. You can read the full side-effect profile for semaglutide on the NHS semaglutide medicines page.

That said, the absence of anxiety from the common-effects list does not mean it cannot happen, and if you are wondering whether Wegovy can cause anxiety, the answer is more nuanced than the prescribing information alone suggests. Clinical trials measure effects that occur at a meaningful frequency across thousands of participants; rarer or more individual responses are captured through post-marketing surveillance rather than trial data. Wegovy carries a Black Triangle (▼) designation from the MHRA precisely because additional monitoring is ongoing. If you want a thorough look at how Wegovy side effects and anxiety are connected, including what patients and researchers have observed beyond the trial data, we have covered that in detail.

The practical upshot: anxiety is not expected, but it is possible, and it deserves attention rather than dismissal if you experience it.

Step 2: Separate the physical from the psychological, because the link is real

Here is something our prescribers hear regularly from patients: anxiety that begins on Wegovy is often not a direct pharmacological effect of semaglutide. It is the body's response to nausea, disrupted eating, or the sudden change in appetite that the medicine produces.

Persistent nausea (which can be significant at the start of treatment or after a dose increase) is physically uncomfortable and can feel destabilising. Eating patterns shift, sometimes dramatically. Some people feel lightheaded if they are not taking in enough calories or fluids. These experiences, stacked together, can produce genuine psychological distress that looks and feels like anxiety. The causes matter because the solutions differ: managing nausea through slower titration or dietary adjustment often resolves the accompanying low mood, whereas a direct medication effect would require a different conversation.

There is also a broader context worth acknowledging. Living with obesity frequently carries its own psychological load, years of diet culture, stigma, and complex relationships with food. Starting a treatment that changes hunger and eating can surface feelings that were already there. For more on the relationship between Wegovy and depression or anxiety together, we have covered that intersection separately.

Step 3: Know when anxiety on semaglutide becomes something to act on immediately

There is a difference between feeling a bit unsettled in the first fortnight of treatment and experiencing sustained or worsening anxiety, low mood, or thoughts of self-harm. The former often settles. The latter requires prompt clinical contact, not a message left for next week's review.

The MHRA asks that patients and healthcare professionals report any suspected mental health side effects through the Yellow Card scheme. This is not just administrative: it is how regulators identify patterns that may not have been detectable in trial populations. If you are already on Wegovy and noticing something, reporting it helps other patients down the line.

The things to treat as urgent: anxiety severe enough to interfere with daily life, sudden low mood or hopelessness, any thoughts of self-harm or harming others, or a feeling that symptoms are escalating rather than settling. Contact your prescriber the same day. If you are in crisis, contact NHS 111 or your GP. At nume, our clinical team is available for aftercare seven days a week, reach our team here if you need to talk to someone about symptoms between reviews.

Step 4: Ask whether anxiety is a reason to stop, or to adjust

Not every experience of anxiety on Wegovy means the medicine is wrong for you. Sometimes slowing the dose escalation, improving hydration, or addressing the nausea resolves the psychological symptoms too. Sometimes a prescriber may judge that a different approach is better.

What you should not do is stop treatment abruptly without clinical guidance, or assume that anxiety during treatment is just something to push through quietly. A conversation with your prescriber is always the right move. If you are thinking about Wegovy and have a history of anxiety or other mental health conditions, the consultation is the place to raise that, it is directly relevant to whether and how the medicine is started. You can learn more about the risks and side effects of Wegovy to prepare for that conversation, or find out how access to Wegovy through a regulated UK pharmacy works. The full Wegovy guide on our site covers eligibility, evidence and the treatment pathway in one place.

If you would like a prescriber to review your specific situation, you can speak to our prescribers through a free consultation. They will take your full history into account before making any recommendation.
