Can You Open a Mounjaro Pen — and Should You?

What you find when the pen arrives, and what you definitely won't

Picture the moment the DPD box arrives, in plain packaging, and you're holding the pen for the first time. It's a squat, rectangular cartridge with a dial that clicks round as you use each dose, a dose window so you can see which number you're on, and a button you press to inject. That is broadly it from the outside. There is no twist-off top, no needle guard you screw on separately, and no glass barrel you can lift out.

The needle assembly sits entirely inside the pen body and only moves into position during injection, specifically, it extends when you push the pen flat against your skin and press the injection button, then retracts when you lift off. This is a deliberate safety feature. The Mounjaro KwikPen's design is engineered so that the sharp never protrudes until the moment it is needed and retracts the instant it is done. Trying to access it manually means forcing apart a plastic casing that is bonded and sealed at manufacture. The result would be a broken pen, a potentially contaminated solution, and a live retracted needle with no protective housing around it.

Nothing inside is designed to be touched by the user. The medicine is factory-loaded, sterile, and sealed. Opening the casing would not give you access to anything useful, it would only render the device unsafe and the medicine unusable.

The four-dose system and what 'used up' actually looks like

Each KwikPen contains tirzepatide solution sufficient for four once-weekly doses. The dose window shows a number from one to four; after each injection the number advances by one. When the fourth dose has been given, the pen locks, the button will not depress again. This is not a fault. It is the pen telling you it is finished.

A common worry is whether the pen still contains medicine after it locks. Residual liquid is visible in most pens even after dose four, and this can cause real anxiety: it looks like waste. The SmPC and Patient Information Leaflet are clear that this residual is intentional, it accounts for the dead space in the mechanism and is not a retrievable dose. There is no way to extract it safely, and attempting to do so (including trying to open the pen to access the cartridge) risks serious harm.

Once locked, the pen goes into a sharps container. If you do not yet have one, a 1-litre sharps bin is available through our pharmacy. Most GP surgeries and local pharmacies will also accept full sharps containers for disposal, worth confirming with yours before it's full.

What to do when the pen seems to malfunction

Occasionally a pen will not inject smoothly. The button may feel stiff, the dose window may appear stuck, or nothing seems to happen when you press. Before reaching for anything to prise the device open, run through the checklist your prescriber or the Patient Information Leaflet gives you.

First, confirm the pen is at the right temperature. A pen taken straight from the fridge (particularly on a cold winter morning before breakfast, when you'd rather just get it done) can be sluggish. Eli Lilly recommends allowing it to reach room temperature for a period before use; the exact time is in the leaflet. Second, check you are pressing the pen fully flat against your skin before pressing the button: a partial contact can prevent the needle from extending properly. Third, confirm the dose window has not already advanced past where you expected, occasionally a dose is delivered without the usual sensation, particularly as technique improves.

If the pen genuinely appears faulty (medicine leaking from the casing, visible damage, or a button that will not move at all on the first dose) do not attempt to fix it yourself. Sharing, transferring, and other things the pen is not designed for

A question that sometimes follows the 'can you open it' query is whether the pen can be shared between two people, splitting doses to reduce cost, for instance. The answer is no, and the reasons go beyond legal ones. The retracted needle mechanism is single-use by design; once it has extended and retracted, its sterility cannot be guaranteed for a second person. Sharing a pen carries a genuine risk of transmitting bloodborne infections. This is not a theoretical concern.

There is also the matter of dosing: tirzepatide treatment starts at 2.5mg and is titrated by the prescriber in line with your individual response and tolerability. A pen calibrated to one person's prescribed dose is not appropriate for another person who has not been assessed.
