Cheaper weight loss injections: what you are actually comparing

You have found two prices. Here is what the difference probably means.

Picture this: you have three browser tabs open. One shows a monthly figure that looks reassuringly modest. Another is notably higher. A third sits somewhere in between. You are trying to work out whether the cheap one is just efficient, or whether something is missing.

In the UK private market, the list price of tirzepatide alone changed sharply from 1 September 2025, when Eli Lilly raised the price of the highest-dose pen from roughly £122 to around £330. That single manufacturer move pulled private clinic prices upward across the board. Reported private prices since then range from approximately £149 to £375 per month depending on the dose and the provider, according to widely reported figures following the Lilly announcement. The variation is real, and it reflects dose differences (lower doses genuinely cost less) but it also reflects structural differences in what providers bundle into that number.

A consultation fee charged separately at each visit, a prescription fee added at checkout, a delivery charge, or a monthly platform fee can each add £15 to £60 to a price that looked competitive at first glance. The comparison that matters is not headline versus headline, it is total monthly outlay, including every step from consultation to delivery.

What a lower price sometimes does not cover

The clinical assessment is the part most easily obscured in a price comparison. Some providers charge a consultation fee on every visit; others fold it in. Some charge for the prescription separately; some include it. Delivery (and whether it is tracked, next-day, and genuinely free) is worth checking explicitly. Aftercare matters too. If your dose needs reviewing, or you have a question on a Sunday afternoon, is that included or does it trigger another charge?

There is also the question of what happens when your dose changes. Titration is a normal part of treatment: most people start at the lowest dose and move upward gradually over months, guided by their prescriber. If each dose-change consultation is billed additionally, a price that looked straightforward at 2.5mg looks different six months later at a higher dose. Comparing injectable weight-loss medicines on cost is useful context if you are weighing up which medicine as well as which provider.

None of this is to say that a higher price is always better. It is to say that the right question is not "which number is smallest?" but "what does this number include, and what will I pay in full over six months?"

For a prescription medicine, price is the wrong measure of value

Weight-loss injections like Mounjaro and Wegovy are prescription-only medicines assessed under UK law. The MHRA has warned publicly about counterfeit weight-loss pens sold online, including fakes that have contained insulin rather than the labelled medicine. Enforcement action in 2025 uncovered around 20 million doses of illegally traded weight-loss medicines worth approximately £45 million, including the first UK manufacturing site producing counterfeit tirzepatide pens. The pattern is consistent: the sellers offering these medicines at the lowest prices, without a proper prescription process, are the highest-risk sources. You can report a suspect seller via the MHRA's Yellow Card scheme.

The NICE technology appraisal for tirzepatide (TA1026) sets out the clinical framework within which these medicines are intended to be used, with appropriate assessment, titration and monitoring. A low price from an unverified source does not come with that framework. A legitimate price (whether from a regulated online pharmacy or an NHS service) does. NHS access to weight-loss injections is possible under specific criteria if you want to explore that route.
