It's a reasonable assumption. The pen contains the same molecule at the same dose wherever you buy it, so the obvious move is to find the lowest price and go there. The problem is that the number in the headline rarely reflects the full cost of receiving treatment safely, and for a prescription medicine it definitely doesn't reflect the only thing that should matter: whether you're getting genuine medicine, prescribed appropriately, by someone who has actually assessed you.
Eli Lilly raised its UK list price for Mounjaro significantly from 1 September 2025. Reported figures show the 15mg pen's list price rising from around £122 to around £330, a change that filtered through private providers at different speeds and in different ways. Some providers absorbed part of the increase; others passed it on in full; a few kept headline prices low by quietly adding fees elsewhere. Understanding what Mounjaro 15mg actually costs means reading past the first number you see.
There is also the supply-chain question. At the 15mg dose (the highest licensed strength) you are deep into a treatment course. The NICE appraisal of tirzepatide, TA1026, notes that clinical progress should be assessed at six months; that assessment requires continuity of prescribing, not a series of one-off purchases from whoever is cheapest that week.
A quoted per-pen price frequently excludes several things that are not optional. Clinical consultation fees are common, some providers charge £20–£50 separately. Prescription fees are another. Delivery charges, particularly for temperature-controlled tracked dispatch, add more. Dose-review support (the clinical check before a prescriber approves your next pen at the same or higher dose) is sometimes bundled, sometimes billed, and sometimes simply absent.
That last point matters most at 15mg. By the time a patient reaches this dose they have titrated through 2.5mg, 5mg, 7.5mg, 10mg and 12.5mg, a process the NHS England wraparound guidance and the NHS medicines page on tirzepatide both describe as requiring ongoing clinical oversight. A prescriber who reviewed you once at the start of treatment and has not checked in since is not providing that oversight, regardless of what the invoice says.

The MHRA has warned publicly about counterfeit weight-loss pens circulating online and through social media. In October 2025 an illicit manufacturing site producing fake tirzepatide pens was raided in Northampton; enforcement in 2025 involved around 20 million doses of illegally traded weight-loss medicines with a street value of approximately £45 million. A small saving per pen is not the measure that keeps a treatment course safe, genuine supply chain, verified identity, and a prescriber who has sight of your current weight and health are.
