How Wegovy's click dose system works — and what each number means

Step 1, Preparing the pen before you inject

Before anything else, check that the pen you're about to use matches the strength on your prescription label. It takes under a minute: hold the pen up to the light and confirm the liquid inside looks clear and colourless to faintly yellow, with no particles or cloudiness. If it looks wrong, do not inject and contact your pharmacy. Each Wegovy pen has already been factory-set to its dose; there is no dial to adjust and no way to change the amount you receive.

Remove the pen cap and attach a new needle, following the instructions in the leaflet. Novo Nordisk recommends priming a new pen before the first use to remove any air from the needle, the leaflet for your specific pen explains whether and how to do this. Once primed and ready, select your injection site (abdomen, thigh or upper arm), clean the area and let it dry.

Step 2, Injecting and counting the click

Press the pen firmly against your skin at a 90-degree angle, then push the injection button down. You will hear and feel clicks as the mechanism releases the dose. Keep the button pressed and the pen held still until the clicks have fully stopped and the flow indicator confirms delivery, lifting the pen before the mechanism finishes means you may not receive the full dose.

The number of clicks varies by pen format and dose strength. Because each pen is pre-filled and factory-set, you are not counting clicks to measure the dose yourself, you are simply confirming that the injection ran to completion. After injecting, hold the pen in place for a count of six before removing it from the skin. Dispose of the needle immediately into a sharps-safe container, reusing needles increases the risk of blocked delivery and skin reactions.

Step 3, Understanding clicks across the dose strengths

The five standard Wegovy strengths (0.25 mg, 0.5 mg, 1.0 mg, 1.7 mg and 2.4 mg) each come in their own pre-filled pen. As the Wegovy click dosage guide explains in more detail, each pen's click mechanism is calibrated to that specific strength, a 0.5 mg pen and a 1.7 mg pen do not work identically even if they look similar on the outside.

For the standard titration path, the NHS describes semaglutide as starting at 0.25 mg for the first month, then escalating roughly every four weeks through 0.5 mg, 1.0 mg and 1.7 mg to reach 2.4 mg as the usual maintenance dose, and patients who have reached that final step can find pen-specific guidance on our Wegovy 2.4 click page. Your prescriber sets each step; you do not change the dose by adjusting clicks. The 0.5 mg pen and the 1.0 mg pen are the two most commonly misunderstood steps in this regard, because patients occasionally wonder whether a different number of clicks would take them to the next dose. It wouldn't, only a new prescription for a higher-strength pen does that.

The 7.2 mg pen approved by the MHRA on 14 April 2026 uses a single-dose auto-dosing mechanism, a built-in covered needle and locks automatically after use. It is licensed only for adults with a BMI of 30 or above and is not the same device as the standard five-strength pens. When the click sounds or feels unusual

Occasionally patients notice the pen feels different, the button sticks, clicks seem to stop early, or nothing seems to have been delivered. These situations matter because semaglutide is a weekly injection: a missed or partial dose can affect how your treatment is going. A few practical points worth knowing: pens stored outside the fridge for longer than the window specified in the leaflet may not deliver reliably; needles left attached between doses can cause air bubbles; and injecting into an area with scar tissue or hardened skin from repeated use in the same spot can affect delivery. Rotating your injection site each week keeps the process consistent.
