Semaglutide works partly by slowing gastric emptying: food leaves your stomach more gradually, which blunts hunger and smooths out blood-sugar responses. That same mechanism extends further down the digestive tract, reducing the speed at which the bowel moves waste along. The result, for a meaningful proportion of people, is constipation. The first set of decisions is dietary. Most people are not eating enough fibre when constipated on Wegovy, partly because reduced appetite means smaller meals, which cuts fibre intake along with everything else. Aiming to keep vegetables, pulses, oats and wholegrains in your meals (even in smaller portions) makes a difference. Hydration matters too. Semaglutide can reduce thirst signals alongside hunger signals, so it is easy to end up mildly dehydrated without noticing. A good practical anchor is drinking a glass of water with every meal and one mid-morning and one mid-afternoon. Nothing complicated.
Gentle physical activity helps stimulate bowel motility. A twenty-minute walk after meals is the most consistently useful suggestion, and it fits alongside the lifestyle support that Wegovy is intended to complement. If dietary and movement adjustments do not resolve things within a week or so, it is reasonable to speak with your prescriber or a pharmacist about a short course of an osmotic laxative. Other signs that warrant a call to your GP or 111 rather than home management: blood in your stool, severe bloating that is getting worse over days, inability to pass wind at all alongside abdominal distension, or constipation accompanied by vomiting repeatedly. These can indicate bowel obstruction or other conditions that need clinical assessment, not dietary tweaks.
If symptoms are less dramatic but are genuinely affecting your day-to-day life, that is still worth raising. A note to your prescribing team costs nothing and can result in a practical change, whether that is a dose adjustment, a pause, or simply reassurance that what you are experiencing is within the expected range.
