Wegovy Makes Me Constipated: What's Actually Going On

Constipation is a well-documented side effect of semaglutide (Wegovy): slowed gastric emptying affects the whole GI tract, not just the stomach.
Most cases are mild to moderate and settle over a few weeks, though they often return briefly after each dose increase.
Staying well hydrated and keeping dietary fibre consistent are practical first steps, but your prescriber should know if symptoms are severe or persistent.
Severe stomach pain, especially pain that radiates towards the back, is a different matter entirely and needs urgent medical assessment.

Constipation on Wegovy is genuinely common, and it tends to catch people off guard — most expect nausea, not the opposite problem. Semaglutide slows the movement of food through the digestive tract, which is part of how it reduces appetite. That same mechanism can make bowel movements slower and harder for some people, particularly in the first few weeks or after a dose increase. It is a recognised side effect listed on the NHS semaglutide medicines page, not a sign that something has gone wrong, but it is worth understanding, and in some cases, it warrants medical attention. Wegovy is a prescription-only medicine; a prescriber assesses suitability and can help you manage side effects throughout treatment.

Managing Constipation on Wegovy: What Helps, What to Watch, and When to Call for Help

The myth worth correcting: constipation means Wegovy isn't working

Plenty of people on semaglutide quietly assume that constipation is a sign something is wrong with their treatment, that the medicine is misbehaving, or that their body is reacting badly. The opposite is usually true. Semaglutide acts on GLP-1 receptors in the gut as well as the brain. One of its key effects is slowing gastric emptying: food leaves the stomach more gradually, which is a significant part of why appetite falls. The same slower transit affects the rest of the digestive tract too. Less frequent bowel movements are the predictable downstream result of a mechanism that is doing exactly what it is supposed to do.

That does not make constipation comfortable or something to just push through without thought. But it reframes it correctly: this is a manageable side effect of a medicine that is working, not evidence of harm. The full side-effect profile of Wegovy covers the broader GI picture, which includes both constipation and, for some people, nausea or diarrhoea, sometimes at different points in treatment.

Constipation tends to be most noticeable in the first four to eight weeks and again after each dose step up. For most people, it settles. If you have been wondering whether Wegovy constipates you and why that happens, knowing that pattern helps, it means there is usually a reason for the timing, and it is not arbitrary.

Practical steps that actually make a difference

Hydration is the most underrated factor. Semaglutide already reduces how much you eat and drink; if fluid intake drops alongside food intake, stools become harder and slower. Aiming for consistent water intake across the day (not just in the morning) is a straightforward place to start. Warm drinks in the morning can also prompt bowel activity for some people.

Fibre matters, but the type matters too. Soluble fibre (oats, lentils, flaxseed, fruits like pears and apples) draws water into the stool and helps it move. Insoluble fibre, eaten without enough fluid, can occasionally make things worse. If your appetite is significantly reduced on Wegovy, your fibre intake may have dropped without you noticing, a small deliberate effort to keep vegetables, legumes and whole grains in meals helps.

Gentle physical movement (a daily walk, for instance) stimulates gut motility in ways that sitting still does not. This does not need to be structured exercise; regular movement across the day is enough to make a measurable difference for most people.

If dietary changes alone are not enough, some people find a stool softener or an osmotic laxative (such as macrogol, which is available over the counter) helpful in the short term. Speak to your pharmacist or prescriber before starting one, particularly if you are on other medicines. The detail on managing constipation specifically on Wegovy covers these options more fully.

When constipation is not the only issue to consider

Most Wegovy-related constipation is uncomfortable but uncomplicated. There are, however, situations where symptoms need prompt attention rather than home management.

The MHRA issued a Drug Safety Update in January 2026 flagging acute pancreatitis as an infrequent but serious risk with GLP-1 medicines. The key symptom to recognise is severe, persistent stomach pain that radiates towards the back, sometimes with vomiting. This is distinct from constipation or ordinary bloating, and it needs emergency assessment. Do not try to manage it at home.

Other situations that warrant a call to your GP or prescriber: constipation that becomes severe and does not ease with simple measures; any rectal bleeding; significant abdominal distension; or constipation combined with vomiting that is preventing you from keeping fluids down. Dehydration from persistent vomiting alongside constipation is a combination that can strain the kidneys, worth raising quickly rather than waiting.

If you experience any suspected side effect from Wegovy that concerns you, you can report it directly at yellowcard.mhra.gov.uk, the MHRA's Yellow Card scheme collects these reports and uses them to monitor medicine safety across the UK. Reporting is open to patients, not just clinicians, and it genuinely contributes to the evidence base.

Our prescribers discuss side effects like this one as a routine part of the consultation process. If you are considering Wegovy or are already on treatment and struggling, the free consultation is a good place to raise it.

A note on timing: dose increases and what to expect

If your constipation arrived or got worse after your dose was stepped up, that timing is not coincidental. The escalating schedule for Wegovy (moving from the starting dose upwards over several months) means the gut's adjustment process repeats itself each time the dose increases. Symptoms that had settled can briefly return.

The pattern is frustrating, but it is predictable. If you find Wegovy is making you constipated again after a dose increase, being deliberate about hydration, keeping fibre intake steady, and moving a little more than usual can help it settle before the next step.

If you are still in the process of deciding whether Wegovy is right for you, the broader Wegovy treatment overview sets out how the medicine works, what the evidence shows, and how clinical assessment works at nume. Our clinical team reviews every consultation personally (the same day) and that includes discussing whether Wegovy makes you constipated and how to manage it with people who are already on treatment elsewhere and thinking about transferring.
