Do You Need to Prime the Mounjaro Pen Every Time You Inject?

Priming is a one-off check done only before the first injection from each new Mounjaro pen — not before every dose.
The purpose of priming is to confirm the pen is working correctly and to remove any air from the needle tip before your first use.
From the second injection onwards, you attach a fresh needle and inject directly, without repeating the priming flow check.
Each Mounjaro KwikPen contains four weekly doses; only the first injection from that pen involves a prime check.

No — you do not prime the Mounjaro KwikPen every time you use it. The pen requires a priming check only before the very first injection from a new pen. After that first use, subsequent injections skip the priming step entirely and go straight to the injection itself. Mounjaro is a prescription-only weight-management medicine; your prescriber and the Patient Information Leaflet guide the process for your specific situation. How priming fits into the Mounjaro injection routine, step by step

Step 1: When you open a brand-new Mounjaro pen

Your Mounjaro KwikPen arrives cold, stored between 2°C and 8°C. Before anything else, allow it to reach room temperature, the Patient Information Leaflet specifies the exact time window, and following that is worth taking seriously. Once the pen is ready, attach a new needle. This is the one occasion where a priming check is part of the process.

Priming means holding the pen with the needle pointing up, tapping gently to move any air bubbles upward, then pressing the injection button until a small drop of liquid appears at the needle tip. That drop tells you the flow path is clear and the pen is working. The full step-by-step priming process covers this in detail if you want to walk through it before your first injection.

One prime check, one drop, done. You do not repeat this for the remaining three doses in that pen.

Step 2: Injections two, three and four from the same pen

For every subsequent dose from the same pen, the routine is simpler. Attach a fresh needle (always a new one for each injection) choose your injection site, clean the skin, and inject. There is no priming flow check in between. The KwikPen injection technique page walks through exactly what those later doses look like in practice.

Rotating sites matters. Abdomen, thigh and upper arm are all suitable; moving the spot each week reduces the chance of localised skin reactions. The Mounjaro injection overview covers site rotation and needle technique in more detail.

Needle hygiene is straightforward but non-negotiable. Remove the needle after each use, dispose of it safely in a sharps container, and never store the pen with a needle attached, this prevents leakage and contamination between doses. If you are going through needles quickly, spare needles for the Mounjaro pen are available separately.

Step 3: Starting a fresh pen the following month

When you open a new pen, the priming check comes back, just once, for that pen. Think of it as a quality confirmation rather than a ritual: you are checking a new device, not re-priming medicine that is already in your system.

Many people find this the stage where small doubts creep in: did I prime correctly, was that drop big enough, what if I missed it? A question our prescribers hear fairly often. The honest answer is that the Patient Information Leaflet describes the check clearly, and if something looks off (no drop appearing, or a much larger-than-expected flow) contacting your prescriber or pharmacy is the right move rather than guessing.

The question of whether priming is required each time is one of the most common technique queries we see, and the answer is consistent across all pen strengths: once per pen, never once per dose.

How this fits the weekly Mounjaro schedule

Mounjaro is injected once a week. Keeping the same day each week helps build the habit, though the flexibility around injection timing is worth understanding too. Because each pen lasts four weeks, most patients will prime a new pen roughly once a month, which keeps the overall process light once the routine is established.

Clinical guidance on Mounjaro (including the injection schedule, storage requirements and what to do if a dose is missed) is summarised on the NHS tirzepatide medicines page, and the full technical detail sits in the Mounjaro SmPC on the eMC. Both are worth bookmarking for reference.

Mounjaro is a prescription-only medicine for adults with a qualifying BMI, prescribed following clinical assessment. If you are already on treatment and have technique questions, our FAQs cover the most common ones, and the support team is available seven days a week. If you are still deciding whether Mounjaro is right for you, checking your eligibility with our prescribers is the place to start.
