The SURMOUNT-1 trial (the pivotal phase 3 study of tirzepatide published in the New England Journal of Medicine) enrolled 2,539 adults with obesity and tracked cardiovascular measures alongside weight outcomes over 72 weeks. Small increases in resting heart rate were recorded across tirzepatide doses, averaging roughly 1–2 bpm at 5mg and approaching 2–4 bpm at the highest doses, compared with placebo. These figures sit within the range seen across the GLP-1 class, and NICE's appraisal of tirzepatide (published December 2024 as TA1026) reviewed the safety data and recommended the medicine for adults meeting eligibility criteria, without singling out heart rate as a barrier to use.
The clinical interpretation matters here. A rise of 2–3 bpm in a group average is statisticaly detectable but below the threshold most cardiologists consider clinically meaningful. Individual responses vary, of course. Some participants experienced no detectable change; a smaller number noticed palpitations, especially in the weeks following a dose step-up. The feeling of a racing heart that comes and goes during titration is the most common presentation, not a sustained tachycardia.

Tirzepatide activates both GLP-1 and GIP receptors, the dual-agonist mechanism that distinguishes it from semaglutide. GLP-1 receptors are found not only in the gut and brain but also on cardiac tissue and in the autonomic nervous system. Most people who notice a slightly elevated pulse on tirzepatide do not need emergency care.
