You started Wegovy, the nausea has settled a bit, but now you find yourself flagging by mid-afternoon in a way you didn't before. It's one of the more common questions people bring to our prescribers. The short answer is: yes, fatigue can be part of the picture with semaglutide, and no, it doesn't mean something has gone wrong.
The semaglutide information on the NHS medicines pages lists fatigue as a known side effect. It tends to follow a familiar pattern: more noticeable in the first few weeks, or in the few days after moving up to a higher dose, and then it softens. That timeline mirrors what happens with the GI symptoms, the body is adjusting to a medicine that changes the pace of gastric emptying and the way appetite signals work.
Eating less is also part of the equation. On a significantly reduced calorie intake, some tiredness is physiologically expected. Getting enough protein and staying hydrated can make a meaningful difference. The GI side effects of Wegovy (nausea, vomiting, loose stools, indigestion) are the most commonly reported, and they quietly drain energy in two ways. First, feeling sick is exhausting in itself; the body is working hard and appetite is suppressed further than the medicine alone would cause. Second, persistent vomiting or diarrhoea leads to dehydration, and dehydration is one of the fastest routes to feeling profoundly fatigued and foggy.
This matters practically. If you're feeling unusually tired, the first question worth asking is whether you've been drinking enough water. Sipping steadily through the day rather than drinking in large amounts at once tends to sit better when nausea is present. If the dehydration is severe (you can't keep fluids down, you feel dizzy standing up, or you're passing very little urine) that warrants urgent medical attention, not a wait-and-see approach. Most fatigue on Wegovy is self-limiting. But there are situations where it deserves closer attention rather than patience.
Severe stomach pain (especially pain that radiates toward the back, with or without vomiting) should prompt same-day medical contact. In January 2026, the MHRA issued a Drug Safety Update reminding prescribers and patients that acute pancreatitis is a recognised but infrequent side effect of GLP-1 medicines; tiredness alongside that kind of pain is not a combination to sit on. Gallbladder problems, which can also occur, sometimes present with fatigue alongside abdominal discomfort.
If fatigue is heavy, doesn't ease after the first couple of weeks at a stable dose, or worsens rather than settles, your prescriber needs to know. There may be a simple explanation (insufficient caloric intake, disrupted sleep, an unrelated health change) or there may be a clinical reason to slow the titration schedule or review the treatment.
