Freeze Dried Tirzepatide: Separating Fact from Fiction

Why the clinical trials used liquid tirzepatide, and what that means for unlicensed powders

The SURMOUNT-1 trial, published in the New England Journal of Medicine, tested tirzepatide in its subcutaneous liquid formulation across 2,539 adults with obesity over 72 weeks. Participants at the 15mg maintenance dose achieved an average body-weight reduction of around 20–21%. That figure is now referenced in NICE's appraisal of tirzepatide (TA1026) as the basis for recommending the medicine. Every milligram of evidence underpinning those results was generated using the liquid KwikPen format, not a reconstituted powder, not a vial, not a freeze dried cake dissolved in a syringe. When someone asks whether a freeze dried version might perform the same way, the honest answer is: there is no data. The stability profile, bioavailability and sterility of an unlicensed reconstituted product are simply unknown.

Pharmaceutical freeze drying (lyophilisation) is a legitimate manufacturing technique used for some medicines that degrade quickly in solution. Tirzepatide as licensed does not require it, Eli Lilly's KwikPen is engineered to be stable under refrigeration between 2°C and 8°C, and the product has a defined room-temperature window described in the Patient Information Leaflet. The format was chosen deliberately. A freeze dried alternative would need its own clinical programme, its own manufacturing validation and its own MHRA approval. None of those exist for any tirzepatide product in the UK.

What the counterfeit market looks like (and why freeze dried formats are appearing in it

A question our prescribers hear most weeks is some version of: "I found tirzepatide powder online for much less) is it the same thing?" It is not. The MHRA has confirmed that illicit tirzepatide products, including fake pens and non-standard formats sold via social media and unregistered websites, are entering the UK market. In October 2025, the MHRA raided the first known illicit UK manufacturing site producing counterfeit tirzepatide, around 20 million doses of illegally traded weight-loss medicines with a street value of roughly £45 million were seized across enforcement operations that year.

Freeze dried or powdered tirzepatide appeals to this market partly because it is easier to ship across borders without cold-chain requirements and harder for customs to identify. That convenience comes at a serious cost. Unlicensed reconstituted products carry unknown concentration, unknown sterility and unknown excipients. Some counterfeit weight-loss products seized by the MHRA have been found to contain insulin, an entirely different medicine with a very narrow therapeutic window. The licensed route: what tirzepatide actually looks like in practice

Mounjaro, the UK brand of tirzepatide, comes in six strengths: 2.5, 5, 7.5, 10, 12.5 and 15mg. Each KwikPen contains four weekly doses as a clear, pre-measured liquid solution. There is no mixing, no measuring and no needles to fill, the pen mechanism handles all of that. Treatment starts at 2.5mg; a prescriber titrates the dose upward based on how well it is tolerated and how the patient is responding. Storage is straightforward: keep the pen refrigerated until you are ready to use it, then follow the room-temperature guidance in the leaflet for the exact window. The one thing that does not happen with a legitimate pen is reconstitution. If a seller asks you to add water to a powder, or supplies a vial and separate syringe rather than a pre-filled device, that product is not licensed Mounjaro. Accessing tirzepatide through a regulated, verifiable pharmacy

Tirzepatide is a Prescription-Only Medicine. The only legal route to it in the UK is a prescription issued following a clinical assessment by a qualified prescriber.
