The most common worry people bring to our clinical team is that semaglutide is doing something harmful to the hair itself, perhaps blocking a nutrient pathway or triggering an immune response. That fear is understandable given how visible the shedding can feel, but the evidence does not support it. Hair loss in this context is classified as telogen effluvium: a disruption to the hair-growth cycle caused by physiological stress, most commonly significant calorie restriction or rapid weight change. The follicles are not being destroyed; they are simply pausing the active growth phase and releasing existing hairs earlier than they would otherwise. Because each follicle operates on its own three-to-five month cycle, the shed hairs represent growth that started months before, which is why shedding often peaks around ten to sixteen weeks after the trigger, and why it can feel alarming even when the underlying cause is already resolving.
Clinical trial data for semaglutide (including the STEP programme trials published in the New England Journal of Medicine) recorded alopecia in a small percentage of participants, consistently higher in the active-treatment group than in placebo, which reflects the telogen-effluvium mechanism rather than a direct drug toxicity. Patients losing weight quickly on a lower calorie intake are the most affected; those titrating slowly and eating adequate protein tend to report less shedding. Wegovy works partly by reducing appetite substantially, which means many people eat far less in the first months of treatment. That reduction is effective for weight loss but can create short-term nutritional stress. Two deficiencies are particularly linked to increased telogen effluvium: inadequate protein intake and low iron (ferritin). Hair follicles have a high metabolic demand; when the body is in a significant calorie deficit, non-essential processes including hair growth can be deprioritised. If you are also low in ferritin (common in women, and easy to miss without a blood test) the shedding can be more pronounced and longer-lasting. Zinc and biotin deficiencies are sometimes cited too, though the evidence for supplementing those specifically is weaker.
The practical implication is that diet quality during weight-loss treatment matters more than most people realise. Aiming for adequate protein at each meal, staying well hydrated, and not cutting calories so aggressively that key nutrients disappear from the diet can all make a difference. A GP or dietitian can check ferritin and advise on protein targets. Most Wegovy-related hair shedding does not need urgent medical attention. It is diffuse (spread across the scalp rather than in patches) and it stabilises once body weight and calorie intake settle at a new equilibrium. For most people the shedding slows noticeably within three to six months of reaching a stable dose, and regrowth follows. That said, there are situations where you should speak to a GP rather than wait it out.
Get medical advice promptly if: hair loss is patchy or comes with scalp redness, scaling or itching (which can point to an unrelated condition such as alopecia areata or scalp inflammation); if shedding is very heavy and persists beyond six months on a stable dose; or if you notice loss from eyebrows, eyelashes or other body hair, which is not consistent with telogen effluvium. A GP can check thyroid function, ferritin and other markers that are straightforward to treat if deficient.
If you believe your hair loss is related to Wegovy, you can report it directly to the MHRA via the Yellow Card scheme. Wegovy carries a Black Triangle (▼) designation, meaning the MHRA actively wants reports of side effects to build the post-market safety picture.
