What the trial cannot fully capture is the subjective texture of that first injection day. Others notice nothing at all for the first 24 hours. There is genuine individual variation, and that variation is not a sign that the medicine is working better or worse.

The timing matters practically too. Many people choose a Friday evening injection so that any first-day grogginess falls over a weekend rather than a working morning. The specific side effects most likely to appear quickly

Nausea tends to be the earliest sensation, sometimes accompanied by a reduced appetite that feels more sudden than expected. When to get medical help without waiting

Most early reactions are uncomfortable rather than dangerous, but some symptoms need same-day or emergency medical attention. The MHRA's Yellow Card scheme exists so that patients and clinicians can report suspected adverse reactions, including serious ones, and the MHRA issued a Drug Safety Update in January 2026 specifically flagging acute pancreatitis as a known, infrequent but potentially serious reaction to GLP-1 medicines.

Contact your GP or call 111 if you develop severe, persistent stomach pain that radiates towards your back, with or without vomiting. That symptom pattern needs assessment for pancreatitis. Gallbladder symptoms (sharp pain in the upper right abdomen, particularly after eating) also warrant prompt review. Signs of a serious allergic reaction (swelling of the face, lips or throat, difficulty breathing, or a widespread rash) are an emergency: call 999.

Significant dehydration from prolonged vomiting or diarrhoea is a risk worth taking seriously. What you can do to make the early phase easier

Practical steps help. Eating smaller portions at mealtimes rather than one large meal reduces the likelihood of nausea after injection. Staying well hydrated (plain water throughout the day, not just alongside meals) supports both comfort and safety. Avoiding rich, fatty or very spicy food in the first week is a straightforward move that many people find genuinely useful; it does not need to be a strict dietary overhaul.

The timing of the injection relative to meals is less critical than many people assume, Wegovy can be taken at any time of day regardless of food, unlike the oral semaglutide tablet.
