STEP 1 followed 1,961 adults with obesity or overweight and at least one weight-related condition over 68 weeks. The most frequently reported side effects across the whole cohort were nausea (around 44% of participants on semaglutide), diarrhoea (around 30%), vomiting (around 24%) and constipation (around 24%). The NHS medicines information for semaglutide lists these same effects, alongside indigestion, stomach pain, burping and fatigue, as the commonly reported ones.
For men reading this, the honest picture is that sex-specific breakdowns were not the primary lens of that trial. What we can say is that the mechanism driving these effects (semaglutide slowing gastric emptying and acting on appetite-regulating pathways in the brain) applies to everyone on the medicine. Most men who experience nausea find it peaks in the days after starting or after a dose step, then eases over a week or two as the body acclimates. If you've come to this page after reading things online, you're probably not the first man to wonder whether GLP-1 medicines affect testosterone, energy or muscle mass in ways specific to male physiology. It's a reasonable question, and it deserves a straight answer rather than a brush-off.
There is currently no robust clinical evidence that semaglutide directly suppresses testosterone in men. Weight loss itself (by whatever means) can influence hormonal markers, and some research suggests that reducing excess body fat may modestly improve testosterone levels in men with obesity. That is not a guaranteed outcome of Wegovy treatment, and it is not something we would present as a benefit; it is simply context. The more relevant muscle-mass point is that any significant caloric reduction carries a risk of lean-tissue loss alongside fat loss. Maintaining adequate protein intake and including resistance exercise alongside treatment is consistently recommended in clinical guidance, though the personal specifics belong in a conversation with your prescriber or a dietitian.

Most side effects from Wegovy are uncomfortable rather than dangerous. A handful are not. The MHRA's Drug Safety Updates for GLP-1 medicines highlight acute pancreatitis as a known, infrequent but serious risk: severe stomach pain that radiates to the back, with or without vomiting, is the warning sign. This requires urgent medical attention, not a wait-and-see approach.
Other situations that warrant prompt contact with a doctor or urgent care include: signs of a serious allergic reaction (swelling of the face, lips or throat, difficulty breathing, a fast heartbeat with dizziness); symptoms of significant dehydration following persistent vomiting or diarrhoea (very dark urine, confusion, muscle cramps); gallbladder-related symptoms such as right-upper-abdominal pain after eating, especially with fever; and any new or worsening low mood, depression or thoughts of self-harm. These are not common — but they are real, and the right response is to seek help rather than to wait for the next scheduled review.
