What the clinical evidence and UK guidance actually tell us about long-term Wegovy use

Is there a set time limit on how long you can take Wegovy?

There is no single universal cut-off written into Wegovy's licence. The medicine is authorised in the UK for adults with a BMI of 30 or above, or 27 and above alongside a weight-related condition such as high blood pressure or obstructive sleep apnoea, and the SmPC does not cap the treatment period for private use. What does shape how long most people stay on it is the NICE appraisal (TA875), which recommends Wegovy on the NHS only within a specialist weight management service and for a maximum of two years, after which NHS funding stops. Private prescriptions are not automatically bound by that two-year ceiling, but responsible prescribers still review ongoing suitability at every repeat, because what was appropriate at month three may look different at month eighteen.

The six-month progress check matters too. NICE's guidance on semaglutide is explicit: if less than 5% of body weight has been lost after six months on the maintenance dose, continuing treatment should be reviewed. That threshold exists because sustained clinical benefit is the justification for any long-term prescription medicine.

What side effects are more relevant the longer you take Wegovy?

The bulk of Wegovy's side effects front-load early in treatment. Nausea, loose stools, constipation, indigestion and fatigue are common during the titration phase, as the body adjusts to each new dose level, and for most people they ease considerably once the dose stabilises. That pattern is well-documented in the STEP 1 trial, where 68 weeks of semaglutide 2.4mg was studied in adults with obesity and the GI side effects were most frequent in the first few months.

Longer-term, the side effect profile is generally more settled but not absent. Two areas the MHRA has highlighted specifically are worth knowing. First, in January 2026 the MHRA issued a Drug Safety Update drawing attention to acute pancreatitis as an infrequent but potentially serious effect of GLP-1 medicines: severe, persistent stomach pain that may spread to the back (with or without vomiting) calls for urgent medical attention, not a wait-and-see approach. Second, gallbladder problems (including gallstones) are associated with significant weight loss by any means, and semaglutide is no exception. If you develop upper-right abdominal pain or pain after eating fatty food, tell your prescriber or GP promptly.

There is also the question of what happens to weight if treatment stops. Evidence consistently shows that body weight tends to return after discontinuation, which is one reason understanding how long it is safe to take Wegovy matters when ongoing prescribing decisions are made carefully rather than treating Wegovy as a short course.

When should you get medical help rather than waiting?

Knowing the difference between an expected adjustment symptom and something that needs urgent attention is the most useful safety knowledge anyone on Wegovy can have. Seek medical help the same day (or call 999 if severe) for: intense, persistent abdominal pain radiating to the back (possible pancreatitis); signs of a serious allergic reaction including facial swelling, difficulty breathing, or a rash spreading rapidly; symptoms of severe dehydration after prolonged vomiting or diarrhoea; and any sudden change in vision if you have a history of diabetic eye disease.

Contact your GP or prescriber more urgently if you notice persistent low mood, thoughts of self-harm, or significant changes in mental health. What does long-term safety research actually look like for semaglutide?

The STEP programme, the largest body of evidence for semaglutide 2.4mg, followed participants for up to 68 weeks. That is meaningful data (thousands of adults with obesity, placebo-controlled, peer-reviewed in the New England Journal of Medicine) but it does not tell the whole story of what happens at year three or five, because those trials have not yet concluded. Regulators are transparent about this: the Black Triangle status on Wegovy's UK licence signals that real-world post-market safety monitoring is ongoing, and healthcare professionals are encouraged to report any new observations.

What we do have is a substantial safety database from the diabetes semaglutide trials (Ozempic has been in use since 2018) and the cardiovascular outcome studies, which together cover years of use in large populations.
