How many injections are in one Mounjaro pen?

Four doses per pen: where does that figure actually come from?

The Mounjaro pen design is specified in the Summary of Product Characteristics approved by the MHRA for the UK market. Each pre-filled KwikPen holds enough tirzepatide solution for exactly four 0.5 ml injections, one per week. That four-dose structure is consistent across every UK strength: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg pens all follow the same four-injection rule. What differs between strengths is the concentration of tirzepatide per 0.5 ml, the volume per injection stays fixed. For context, the SURMOUNT-1 trial (which enrolled over 2,500 adults with obesity and followed them for 72 weeks) used this same pen format throughout. Reading the pen before and after each injection

A question our prescribers hear most weeks is whether patients can tell how many injections they have left. The KwikPen has a built-in dose counter on the side. Before you inject, turn the pen toward the window and check the number displayed. A full pen shows 4; after your first injection it reads 3, then 2, then 1. When the counter reaches 0 the pen is empty and the mechanism locks, it will not fire a fifth time.

This matters practically. If you are travelling or approaching a reorder point, checking the counter at the start of each week prevents a situation where you believe a pen holds another dose when it does not. There is also a separate click mechanism during the injection itself, the pen clicks when you press the button and again when the dose is complete. Does every strength come with the same number of pens?

In UK private supply, Mounjaro is dispensed one pen at a time, that is one four-week supply per order. You do not receive a box of multiple pens unless your prescriber and dispensing arrangement specifically supports that. It is worth being clear on one practical point: a new pen for each strength. When your prescriber moves you from, say, 5 mg to 7.5 mg, your next pen is a 7.5 mg pen, you do not continue the old pen at a lower count. Each pen is factory-set to one strength. The four-injection total resets with every new pen.

Safe disposal after the fourth injection

Once the counter shows 0, the pen should go into an approved sharps bin rather than the household bin or recycling. Used pens contain a retractable needle and residual medicine, both reasons to handle disposal carefully. Many GP surgeries and community pharmacies accept sharps bins for collection; check with your local service. Replacement needles are changed before each of the four injections. Reusing a needle blunts the tip and can cause more discomfort at the site, worth knowing if you have noticed the injection feeling different towards the end of a pen. Some people find Mounjaro injections almost imperceptible, and consistent needle changes are part of why. Fresh needles at each of the four injections is the standard clinical advice.
